About this role

Job summary

We're looking for motivated individuals to join our Paediatric Diabetes Team, supporting children and young people across hospital and community settings. You'll play a key role in delivering specialist care, helping families implement and manage diabetes technology, and providing hands-on support in clinics, schools, and homes.

This vacancy is for a Family support worker on each site- Doncaster and Bassetlaw both on a fixed term basis

Strong IT skills and experience in paediatric care are essential, along with the ability to communicate effectively--especially in languages spoken within the local community. You'll assist with device downloads, troubleshoot tech issues, and help set up drop-in clinics. Your work will also involve supporting emotional and social development through therapeutic activities.

This role requires excellent communication, sound judgement, and the ability to work across professional boundaries to educate and empower families throughout their diabetes journey.

Main duties of the job

The post holder will work closely with children and young people, helping them actively engage with their diabetes care. This role involves working across both hospital and community settings, offering personalised support to patients and their families.

A key part of the role is to motivate and empower children, parents, and carers to improve their health and wellbeing. You will work as a diabetes family support workers and identify families from the low income areas who are not currently on CGM or pump technology and work with the CYA and their families to find creative solutions to remove barriers. Working in this role you will help reduce health inequalities and improve outcomes for children and young adults with diabetes

In clinic, the post holder will assist with downloading insulin pumps and other diabetes technology, providing timely feedback on any issues or concerns to support effective care and decision-making.

Qualifications & Training Essential

Educated to GCSE (or equivalent) standard, with a pass in English and Maths, NVQ Level 2 or 3 in Care or equivalent Hold Full UK driving license and have access to own vehicle to be used for work Evidence of recent learning or development in health

IT qualification

Knowledge & Experience Essential

Significant experience of working with children, young adults and families Experience in designing and running a range of patients engagement activities

Desirable

Experience as a diabetes family support worker Experience in working with children, young people, or families in a health, social care, or community setting Knowledge of paediatric diabetes technology

Personal Attributes & Skills Essential

Demonstrate good verbal communication skills with colleagues, parents and children & young adults
