Job summary

We are looking for a hardworking, team player to join our Nursing team. You must be registered with the NMC and ideally have experience working in General Practice or a willingness to undertake training to achieve the skills needed.

We would consider any number of hours and outside interests are supported.

Main duties of the job

As a Practice Nurse you will provide high levels of nursing care to meet the needs of patients within the limitations of the Primary Care setting. Main duties will include:

About us

Our practice is one of the biggest in Coventry with a list of 25,000 patients. We are based at Engleton House Surgery, in the Northwest of the city, with a branch surgery on campus at Coventry University. This gives us a mix of patient ages which increases the variety and range of our work. Working at the University also gives a change ofs cenery during the week.

We have a large multidisciplinary team of 18 doctors, 2 practice nurses, 3 nurse prescribers and an HCA as well as pharmacists, occupational therapists, dieticians and a large admin team.

We are a training practice dedicated to training the future nursing and medical workforce.

Job description Job responsibilities

The Practice Nurse will be responsible for delivery of high quality, evidence-based nursing care to the practice population. Supported by the Lead Practice Nurse and supervising any less qualified clinical colleagues, they will deliver care within the boundaries of their role, focusing on health promotion, patient education, assessment, screening, prevention, treatment and monitoring of long term health conditions. They will work collaboratively with the general practice team to meet the health needs of the patients, supporting the delivery of policies, procedures and services to meet practice goals.

This job description outlines the duties as currently required but may be amended to reflect future developments in the service and the impact of new clinical evidence or technology on the role.

MAIN DUTIES AND JOB RESPONSIBILITIES

Clinical Patient Care

Nursing care

To provide high levels of nursing care to meet the needs of patients within the limitations of the primary care setting.

To support GP colleagues in the care of patients requiring medical management prior to transfer to hospital.

To support management of patients requiring emergency treatment eg resuscitation or anaphylactic shock.

Promotion of healthy lifestyle

To support patients in applying principles of self care and signposting them to the appropriate channels where required.

To provide patient health promotion/ disease prevention education and counselling, focusing on health behaviour change.

To refer patients to in house and local services for advice in healthy eating, smoking cessation, reduction of alcohol and physical activity.

To support practice initiatives to encourage patient participation in healthy living.

Wound management

To provide care for all wounds, including assessment and management, suture/clip removal, photography, dressings, treatment and patient advice.

To liaise where appropriate with GP colleagues, tissue viability, community and secondary care services.

Contraception management

To assess and manage patients requiring advice and treatment for contraception.

To ensure patients are monitored appropriately depending on their chosen method of contraception.

To manage patients attending for emergency contraception.

To liaise with GP colleagues as appropriate.

Cervical screening

To advise women and carry out cervical cytology/HPV sampling.

To support provision of an adhoc service offering an opportunity for hard to reach patients to have their smears.

Immunisation for UK schedule and at risk conditions

To advise patients and administer recommended vaccines by PGD or PSD as per the UK schedule and dependent on any significant health conditions.

To support the annual flu campaign and other mass population vaccination programmes.

Management of long term conditions asthma, COPD, diabetes, hypertension, CHD, stroke, CKD, mental health, learning disability and dementia.

To assist GP's in the management of patients with chronic diseases. This will include assessment, arranging appropriate investigations and monitoring, identifying significant abnormalities, developing individual management plans according to practice policy and national guidelines. This will also include offering patient specific health advice regarding individual conditions and referral to outside agencies as required.

To support the practice to achieve targets as detailed by the Quality and Outcomes Framework.

Therapeutic injections

To administer therapeutic injections for patients under an individualised prescription or patient specific direction.

Investigatory procedures and sample transportation

To take MSU, urine acr, HVS, chlamydia or body part swabs as required by GP or practice protocol.

To label correctly, store and despatch safely all laboratory specimens including blood, urine, swabs, smears, stools.

Clinician support

To act as a chaperone or similar support for other colleagues as needed.

To assist in the provision of procedures such as minor surgery, insertion of contraceptive implant, IUCD.

Clinical Environment

Clinical rooms, stock and equipment

Ensure all clinical rooms are kept tidy, well stocked, clinically clean and free from clinical waste.

Ensure all medical equipment is fully working, clean and calibrated as required.

Endeavour to ensure that all necessary medical equipment is kept in all required areas.

Report any low levels, requests, damage or concerns regarding stock or equipment to Lead Nurse or Practice Manager.

Ensure all clinical stock is kept at a working level and within expiry date.

Ensure ordering of stock or equipment is reported to Lead Nurse or Practice Manager.

Ensure any deliveries of stock or equipment are dealt with as timely as possible.

Clinical drugs and vaccines

Operate a system to ensure regular rotation and review of stock.

Ensure all drug and vaccine stock are kept at a working level and within expiry date.

Ensure cold chain policy is adhered to at all times and report any deviations to Lead Practice Nurse or Practice Manager.

Report any low levels of stock to Lead Nurse or Practice Manager.

Ordering of stock to be reported to Lead Nurse or Practice Manager.

Emergency equipment

Ensure all emergency equipment is checked and signed for monthly in Emergency Checks folder.

Ensure all emergency equipment and drugs are replaced after each use.

Report any defects or concerns to the Lead Nurse or Practice Manager.

Administrative

The Practice Nurse will:

Ensure accurate, contemporaneous notes of all consultations, treatments and any significant administrative details are recorded in the patients emis or paper notes as appropriate.

Ensure any clinical or administrative tasks are responded to promptly and appropriately.

Support the practice in the delivery of nationally and locally commissioned enhanced services eg treatment room, pertussis, seasonal influenza, shingles and pneumococcal vaccination.

Participate in audit where appropriate.

Co-operate and participate as required in any research projects within the practice.

Assist the Lead Nurse in monitoring standards of care, clinical searches and formulation of clinical procedures/protocols/templates etc as required.

Ensure that patient queries and complaints are dealt with promptly and according to practice policy.

Professional

The Practice Nurse will:

Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC).

Participate in any training programme implemented by the practice as part of their employment including CPR, safeguarding, cervical cytology, immunisation, anaphylaxis and infection control.

Continually assess own learning needs and make effective use of learning opportunities both inside and outside the workplace. Reflect on learning and feedback relevant information to peers about current and future developments.

Successfully complete NMC revalidation every 3 years.

Participate in an annual individual performance review to aid professional development and support service requirements.

Communication

The Practice Nurse should recognise the importance of communication and will strive to communicate effectively and appropriately with all other members of the practice team, patients, carers and any outside agencies.

Team working

The Practice Nurse will work as an effective and responsible team member. She will respect and support colleagues, delegate and accept delegation appropriately, prioritise own workload and effectively manage own time. She will also utilise referral mechanisms within the practice and collaborate with colleagues in clearly defined values, direction and policies that impact on delivery of patient care.

All accessible information relating to patients and their carers, practice staff, other healthcare workers and the practice as a business organisation, is regarded as strictly confidential. Any information may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.

Risk management

The Practice Nurse will manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients. This will include:monitoring work areas and practices to ensure they are safe and free from hazards, applying infection control measures according to local and national guidelines, ensuring safe storage, rotation and disposal of vaccines and drugs, undertaking mandatory and statutory training, supporting maintenance, calibration and PAT testing of equipment, awareness and utilisation of panic alarm/security door system. This also includes recognising and reporting issues related to substance abuse, domestic violence; child protection; sexual exploitation, vulnerable adults and social problems.
