Job summary
An exciting opportunity has arisen for a motivated qualified HCPC registered physiotherapist to join our friendly, driven and passionate Cardiac Rehabilitation Team at Kingston and Richmond NHS Foundation Trust. You will provide a specialist rehabilitation service to patients with a range of cardiac conditions across the organisation within outpatient clinics, Cardiac Rehabilitation exercise classes and Heart Health education sessions. You will support and advise patients and their families during their pathway through the service. You will be expected to work across various Trust sites within the Borough of Richmond as our classes and clinics are at different locations requiring travelling between sites.
Main duties of the job
o To manage own caseload, assess, plan, implement and evaluate interventions using evidence based/best practice, working towards patient centred goals in the context of the speciality and as part of the multi-disciplinary team.
o To be accountable for their own individual decisions and professional actions.
o To participate and flexibly assist senior colleagues in the wider service needs and development.
o Comply with the Chartered Society of Physiotherapy Care Standards of Practice and Health Professionals Council Standards of Performance Conduct and Ethics of all other relevant national and local policies and procedures.
o To assist the Team Lead in the provision of the Cardiac Rehabilitation Service as necessary
