Job summary
Pathfields Practice Network is looking for an experienced and motivated Practice Nurse to join our friendly and supportive multidisciplinary team across our Plymouth sites.
This role would particularly suit a nurse with experience or a strong interest in chronic disease management, including Asthma, COPD, and Diabetes, who is passionate about improving patient outcomes and delivering high-quality, person-centred care for individuals living with long-term conditions.
You will play a key role in delivering high-quality primary care services, supporting patients with long-term conditions and promoting preventative healthcare within our community.
The successful candidate will be supported by an experienced Lead Nurse who provides clinical leadership, mentorship, and guidance to the nursing team, helping to ensure high standards of patient care and ongoing professional development.
Working Pattern
18 hours per week

Key Responsibilities
The Practice Nurse will support the delivery of a range of clinical services including:
Chronic disease management clinics, including Asthma, COPD, and Diabetes Health promotion and preventative healthcare Supporting patients with long-term conditions through education and care planning Delivering evidence-based nursing care within primary care guidelines Working collaboratively with the wider multidisciplinary team Maintaining accurate clinical records using NHS clinical systems Supporting quality improvement initiatives and patient safety standards
Remote duties, where appropriate, may include chronic disease reviews, patient education, care planning, and virtual consultations.

Person Specification
Qualifications Essential
Registered General Nurse (RGN) with current NMC registration Experience working within Primary Care or General Practice Knowledge and experience in chronic disease management Excellent communication and interpersonal skills Ability to work independently and as part of a multidisciplinary team Strong organisational and time management skills Commitment to patient-centred care and continuous professional development
Desirable
Knowledge of EMIS or SystmOne clinical systems
Experience Essential
Preferred Skills and Experience (training and development can be supported) Experience working in primary care as a Practice Nurse Experience managing chronic disease and long-term condition reviews Experience delivering immunisations and vaccination programmes Experience in, or an interest in developing skills in, Learning Disability health reviews and assessments Understanding of clinical governance, evidence-based practice, and Patient Group Directions (PGDs)
