Job summary
We are seeking a dedicated and motivated Practice Nurse to join our friendly and enthusiastic team of Nurses and Healthcare Assistants. The successful applicant will play a key role in delivering high quality nursing care to our patients and supporting the ongoing development of our clinical services.
Working part time, 2-3 days per week, you will provide a wide range of Practice Nurse duties, including:
Chronic Disease Management (Diabetes, COPD, Asthma)Experience of QOF and an understanding of achieving clinical indicatorsChildhood and Travel ImmunisationsCervical ScreeningDressing and Wound CareContraception servicesECG recordingHealth promotion, lifestyle advice, and signposting to relevant services when appropriate
This is an excellent opportunity for an experienced Practice Nurse or a registered nurse looking to develop their skills within primary care. You will be supported by a welcoming multidisciplinary team committed to providing safe, effective, and compassionate care.
Main duties of the job
As a Practice Nurse, you will be responsible for delivering high quality general nursing services and chronic disease management to our patients. Working collaboratively within our multidisciplinary team, you will help meet the needs of our population, support improved health outcomes, and contribute to maintaining the practices performance objectives.
Duties and Responsibilities include:
Applying a comprehensive understanding of chronic disease management.Monitoring and reviewing patients with chronic conditions and comorbidities.Providing advice and support to promote effective self-management, following practice protocols and local formularies.Contributing to the development of practice protocols, audits, and quality improvement activities.Keeping clinical knowledge and skills up to date in accordance with current guidelines.Delivering general practice nursing services including cervical screening, childhood immunisations, travel vaccinations, wound and ulcer care, and venepuncture.Assisting and performing routine patient care tasks as directed by senior nurses and GPs.Supporting a positive, collaborative, and healthy working environment.

Job Summary
The post holder is a caring, committed, and experienced registered nurse who, acting within their professional boundaries, will provide high quality care for patients from initial history taking through to clinical assessment, diagnosis, treatment, and evaluation. They will demonstrate safe and competent clinical decision making, delivering expert nursing care including assessment, diagnostic skills, and prescribing where applicable, within the general practice environment.
