Job summary

Pathfields Medical Group is looking for a Practice Nurse to join our friendly, forward-thinking, and supportive multidisciplinary team.

This role would particularly suit a Practice Nurse with experience with a strong interest in Learning Disability health reviews and chronic disease management. However, we are equally keen to hear from candidates who are motivated to develop these skills, and we will provide the support and training needed to help you achieve this. We are looking for someone who is passionate about improving outcomes for patients with complex needs and reducing health inequalities within our community.

You will play a key role in delivering high-quality primary care services, supporting patients with long-term conditions and promoting preventative healthcare.

Main duties of the job

Key Responsibilities

The Practice Nurse will support the delivery of a range of clinical services including-

Chronic disease management clinics (e.g. Diabetes / Asthma / COPD)

Learning Disability health checks reviews and assessments

Immunisation and vaccination programmes (adult and childhood)

Womens health services including cervical cytology and contraception advice

Wound care including doppler assessments

Working Pattern

29.5 hours per week

Monday: 08:00 18:00 (30 min lunch)

Tuesday: 08:00 18:00 (30 min lunch)

Wednesday: 08:00 13:00

Thursday: Non-working day

Friday / Saturday: (alternate weeks Friday one week, Saturday the following week)

Person Specification

Qualifications Essential

Registered Nurse with current NMC registration

Experience Essential

Experience working in primary care as a Practice Nurse Experience managing chronic disease and long-term condition reviews Experience delivering immunisations and vaccination programmes Experience in, or an interest in developing skills in, Learning Disability health reviews and assessments Understanding of clinical governance, evidence-based practice, and Patient Group Directions (PGDs) Personal Attributes Strong communication and teamwork skills Ability to work independently while contributing positively to the wider multidisciplinary team
