Job summary

We are looking to recruit a Newly Qualified Nurse, or an experienced nurse wishing to pursue a career in general practice, providing health and wellbeing advice, care and treatment to our patients. This post is a training opportunity supported by the VTS programme with South Yorkshire Primary Care, Workforce and Training Hub.

Main duties of the job

Our practice population has a wide variety of needs, from management of complex and chronic conditions to health screening and advice, immunisation and vaccinations, and identification and treatment of all common health concerns.

This is a permanent post with hours to be negotiated with a maximum of 30 hours per week between the hours of 7 am Monday to 6 pm Friday. The rate of pay will be dependant on experience.

Job description Job responsibilities

The post holder is responsible for the delivery of practice nursing services care to the population the practice serves. Supported by senior nurses within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring long-term conditions, and engaging in health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing nurse leadership as required.

Person Specification

Interests Essential

Developing a long term career in Primary Care. Developing enhanced and additional skills. The wellbeing of the local community.

Desirable

Interest in undertaking enhanced roles e.g. non-medical prescribing. Interest in supervising and supporting learners in practice including student nurses.

Experience Essential

Working inter-professionally. Using electronic healthcare records systems. Awareness and adherence to infection control standards. Understanding of the principles of Safeguarding.

Desirable

Having worked with other members of the Primary Care Team (Community Pharmacists, Dietitians etc.) Minor illness management Triage in healthcare. Knowledge and understanding of current national developments and trends in health care. Experience of multi-skilled approach to Chronic Disease Management.

Qualifications Essential

Registered Nurse (with a current NMC PIN or one pending). Hold (or about to be awarded) a first level degree in nursing. Wellbeing and Health Promotion Awareness of the role of community nursing overall. Knowledge of quality assurance processes. Willingness to participate in the VTS programme with South Yorkshire Primary Care, Workforce and Training Hub.

Desirable

Understanding of the General Practice environment. Knowledge of the audit process in healthcare Awareness of issues across the wider health economy.

Skills / Competencies Essential

Able to work alone without direct supervision when appropriate. Able to undertake and report on autonomous decisions taken in practice. Excellent interpersonal skills. Excellent communications skills. IT skills. Time management. Problem solving.

Desirable

Additional skills in areas key to General Practice e.g. phlebotomy, immunisations and vaccination, wound management, health screening, travel health advice. Use of primary care clinical systems including SystmOne, TPP and EMIS including audit and report writing.
