Community Practitioner - Discharge to Assess Services
Band 6 - 37.50hrs/week - Permanent
The Discharge to Assess team is an innovative and exciting service, aiming to facilitate timely discharges to the residents of Stockport and surrounding areas. Working as part of a multi-disciplinary team, we will facilitate timely discharges from acute hospital sites and provide assessment and support planning to meet a person's needs in the community. The post holder will provide and/or arrange appropriate support interventions in the community via Discharge to Assess collaboratively working closely with colleagues across all sectors.
Working as part of a lively and dynamic multi-professional team, you will deliver holistic support and interventions to improve the health and well-being of individuals in a bed based facility. Main duties of the job
We are looking for highly motivated, forward thinking and experienced therapist with experience in community settings who will work alongside an integrated Multidisciplinary Team to improve flow and quality of the patient's journey. You must have excellent communication, organisational and leadership skills, with the ability to work autonomously, whilst being flexible and adaptable to supporting the service leads in developing and changing the service as it evolves.
You will be part of an integrated therapy team and we have a strong commitment to support all staff members with CPD opportunities. We offer regular supervision and annual appraisal with a regular in-service training programme.
The post requires a commitment to weekend working which is underpinned by teaching and a competency programme.
The post requires working hours over a 7 day service and fulfilling the service needs until 9pm.
