About this role
Job summary
You will be responsible for dealing with around 30 patient contacts per day using a range of consultation methods including face to face, telephone and video consults.
You will be an independent prescriber and up to date with current presribing guidance and NICE guidelines.
You will independently assess, take history and diagnose a range of ongoing and acute presentations as part of a multi-disciplinary team.
Main duties of the job
30 patient contacts per day.
Fair share of pathology to review, action and file.
Independent prescriber.
Onward referrals into secondary care.
Home visits where required.
Care and Nursing Home Weekly Ward roud where required.

Person Specification
Experience Essential
3 years General Practice experience or equivalent.
Qualifications Essential
Masters or equivalent in Advanced Clinical Practice Independent Prescriber
