Job summary

We are seeking an experienced and compassionate Advanced Clinical Practitioner (ACP) to join our Special Allocations Scheme (SAS) service.

The SAS service provides essential primary care support for patients who have been removed from mainstream GP practices due to challenging behaviours. Many patients have complex health and social needs, including trauma backgrounds, making this a highly rewarding opportunity to deliver meaningful, person-centred care and support rehabilitation.

This role offers the chance to work autonomously while also being part of a supportive multidisciplinary team, delivering both remote triage and face-to-face consultations.

The service operates Monday to Friday between 08:00 and 18:30hrs.

Hours: 5 10 hours per week (1 2 shifts). Working days and hours negotiable.

Main duties of the job

As an ACP within the SAS service, you will:

Provide first-contact assessment, diagnosis, and management of undifferentiated physical and mental health presentations Deliver remote triage and face-to-face consultations using advanced clinical assessment and decision-making skills Manage acute presentations, long-term conditions, and preventative healthcare interventions Prescribe safely and appropriately within scope of practice Request and interpret investigations to support clinical decisions Deliver holistic, patient-centred care considering physical, psychological, social, and cultural needs Identify and respond appropriately to safeguarding concerns Work collaboratively with multidisciplinary teams and external healthcare providers Maintain accurate, confidential clinical records in line with GDPR and organisational policies Participate in clinical governance, audit, and quality improvement initiatives Support Quality and Outcomes Framework (QoF) targets while maintaining high standards of patient care

Job description Job responsibilities

As an Advanced Clinical Practitioner with the Special Allocations Scheme (SAS), youll play a vital role in delivering high-quality care to our patients. Working both independently and as part of a small team, you will provide expert remote triage and face-to-face consultations, ensuring safe, informed clinical decisions are made with compassion and respect.

The Special Allocations Scheme has been designed to provide primary care for patients who have been removed from mainstream surgeries due to challenging behaviour. This is a fantastic opportunity to support patients with complex needs and often traumatic backgrounds, helping to provide ongoing care, support, and rehabilitation.

Your Key Responsibilities will be to:

- Provide first-contact assessment, diagnosis, and management of patients presenting with undifferentiated physical and psychological health conditions, including emergency presentations and long-term condition management.

- Undertake remote triage and face-to-face consultations, using advanced clinical assessment, history-taking, physical examination, and decision-making skills.

- Deliver preventative healthcare interventions, including health promotion, vaccinations, screening programmes, and patient education.

- Prescribe safely and appropriately within scope of practice, in line with current clinical guidelines and best practice standards.Request and interpret appropriate investigations to support clinical decision-making.

- Deliver holistic, patient-centred care, considering physical, psychological, social, and cultural factors in assessment and treatment planning.

- Identify and respond appropriately to safeguarding concerns, taking timely action in accordance with safeguarding policies and procedures.

- Maintain continuity of care through effective management of long term conditions and collaborative working with multidisciplinary teams and external healthcare providers.

- Recognise when referral or escalation is required and coordinate care effectively with specialist and community services.

- Maintain accurate, contemporaneous, and confidential patient records in accordance with GDPR and organisational policies.

- Participate in clinical governance activities, including audit, quality improvement, and service development initiatives.

- Maintain professional competence through ongoing continuing professional development and adherence to relevant professional standards.

- Support the achievement of Quality and Outcomes Framework (QoF) targets and key performance indicators while maintaining patient safety and quality of care.

- Work collaboratively with clinical and non-clinical colleagues to support the effective day-to-day operation of the service, including attendance at practice and organisational meetings.

Person Specification

Qualifications Essential

- Registered with the NMC (Nursing & Midwifery Council) or HCPC (Health & Care Professions Council) - Be an independent prescriber on the NMC register - At least 6 years post qualification - Completion of an approved MSc Advance Practice Pathway or equivalent, including a Non Medical Prescribing module. - Certified in Level 3 safeguarding for adults and children.

Desirable

- Holds a recognised qualification in teaching and mentorship.

Experience Essential

- At least 3 years of experience working in a primary care GP practice OR in an urgent care setting such as an Urgent Treatment Centre or Emergency Department. - Experience in Primary Care and QoF (Quality and Outcomes Framework) Targets.

Desirable

- Proven experience working in acute care settings and/or primary care environments.

Skills & Abilities Essential

- Strong ability to manage, prioritise, and organise workload effectively.

Desirable

- Skilled in using clinical IT systems efficiently, such as Adastra, Electronic Prescribing (EPS), EMiS, System One.
