Job summary Could you help remove barriers to healthcare and improve outcomes for adults with a learning disability across the Island?
As Learning Disability Liaison Nurse - Adults, youll support individuals to access mainstream health services, ensuring inclusive, personcentred care across primary, secondary and community settings. Youll work closely with professionals, families and partner organisations to coordinate care and improve patient experience.
Main duties of the job Youll provide specialist advice, support complex cases, develop pathways and deliver training that strengthens practice across services. The role also includes contributing to service development, influencing policy and promoting equal access to healthcare.
