About the Practice/Department/Team
Lakeside St Neots is in a lovely town location with around 23,000 patients. We are a diverse and dynamic team of GPs, Pharmacists, Physios, Social Prescribers, Practice Nurses, Health Care Assistants, Nursing Associates, Emergency Care Practitioners and ANPs.
Job description Job responsibilities
The purpose of the post is to deliver safe, high quality primary care nursing services to the practice population. Working within the NMC Code of Conduct and professional boundaries, the post holder will deliver primary health care interventions and care. The post holder will support service development, the safe delivery of policy and procedures, and demonstrate effective professional nurse standards, advocacy and performance.

Registered Nurse NMC 1st level Registration Evidence of continued professional development

Primary Care Nursing qualification Accredited Diploma Level Award in chronic disease management Mentorship course
Experience Essential
Willingness to undertake training and education to achieve core skills and experience required of a Practice Nurse Experience of managing/dealing with the public/patients Experience of working within healthcare governance systems and understands role and responsibilities for CQC and within risk management Experience of using clinical systems & Microsoft Office software
Desirable
Experience in Primary or Community Care Nursing Management of long term conditions Involvement in implementing and using protocols and clinical guidelines Experience of audit and/or research Experience of SystmOne
