Main duties of the job

As an experienced Practice Nurse the post-holder will work closely with medical and nursing colleagues to provide frontline nursing care to patients. This involves supporting patients to be healthy, monitoring and managing long-term conditions, health promotion and routine screening. They will work collaboratively within the multidisciplinary team in order to meet the needs of the practice population. The nurse will work unsupervised ensuring best practice is provided through continued education and supervision support, and will adhere to policies and procedures.

Working with GPs in the practice the post holder will continually review and evaluate services provided and implement improvements as required.

About us

Abbey Medical Practice looks after 28,600 patients in Wellingborough and surrounding villages.

The Practice is led by seven partners and managed by our Practice Manager. The practice has 6 additional GPs in salaried roles with a Healthy Minds Practitioner and a Clinical Pharmacist in post. We are a training practice for trainee doctors and also a teaching practice for medical students from Cambridge and Leicester.

We recognise the importance of providing support to our clinical staff and have a GP who is available to assist any clinician with clinical queries at all times. This is appreciated by the nursing team.

The practice is very sociable and have regular quizzes and sporting events to have a good work-life balance. In summary, we work hard and we play harder!

Job description Job responsibilities

Role Summary:

As a Practice Nurse the post-holder will work closely with medical and nursing colleagues to provide frontline nursing care to patients. The Post holder will work within their sphere of competence and in accordance with the NMC Code of Conduct at all times. This involves supporting patients to be healthy, monitoring and managing long-term conditions, health promotion, routine screening, administration of injections and wound management. They will work collaboratively within the multidisciplinary team in order to meet the needs of the practice population. The nurse will work unsupervised ensuring best practice is provided through continued education and supervision support and will adhere to policies and procedures.

Working with GPs in the practice the post holder will continually review and evaluate services provided and implement improvements as required.

OVERVIEW OF RESPONSIBILITIES

Key Areas:

Clinical

Assess, plan, develop, implement and evaluate programmes to promote health and well - being.

Manage the care requirements of patients with Long Term Conditions making sure patients are seen promptly, reviewed appropriately and all best practice, QoF and other requirements are met throughout the year.

Be aware of the impact of chronic illness on carers, and help to identify sources of support, assisting in the maintenance of the practice carers register.

Promote patient independence and self care where appropriate.

In liaison with Pharmacists/ANPs and GPs undertake regular patient reviews to meet best practice guidance, QoF requirements and support optimum treatment.

Prioritise health problems and intervene appropriately.

Support patients to adopt health promotion strategies that encourage healthier lifestyles.

Support and manage the health needs of women attending for cervical cytology and sexual health screening

Implement and participate in immunisation programmes for adults and children.

Promote and deliver evidence based care for the practice population.

Administer injections as per patient group directives.

Undertaking treatment area clinical work such as wound dressings including but not restricted to compression dressings, removal of sutures and clips, ECGs, Dopplers, phlebotomy subject to appropriate training and assessment where applicable.

Undertake the collection of pathological specimens, including intravenous blood samples, swabs, smears etc.

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

Act as a chaperone as required by medical colleagues.

Assist as requested where emergency treatment is required for a patient, including CPR

As required by the Nursing Manager, ensure all equipment checks at both practice sites, such as vaccine stock control, cold chain maintenance (all fridge temperatures), emergency trollies and drug stocks are undertaken.

Respond to complaints and clinical incidents as appropriate to ensure robust systems are in place for improving practice through joint learning.

Participate in new initiatives to assist patient diagnosis and treatment, as agreed by practice GPs.

Working with the Nursing Manager develop, implement and evaluate practice protocols, to facilitating change in practice which improves clinical outcomes and meets the needs of patients and carers.

Working with the Nursing Manager and Infection Control Lead ensure that all infection control practice and monitoring aspects in the practice are up to date to meet best practice and regulatory requirements at both practice sites. This will include for example, the correct use and disposal of clinical waste, the correct use and disposal of PPE, the cleaning of body fluid spillages using appropriate PPE, the safe disposal of sharps and full sharps bins. Reporting to the Nursing Administrator any cleaning concerns that may arise.

Liaison with the wider multidisciplinary team as required to ensure patient care is holistic and optimised. In conjunction with the nursing team and other healthcare professionals, develop and regularly review patient information using a variety of media that take into account cultural diversity and communication difficulties.

Promote patient and carer advocacy, ensuring that patients are able to make informed choices wherever possible. Oversee the duties undertaken by the HCAs and assist in training and mentoring as and when required. Other clinical duties as directed and commensurate with role and experience

Non Clinical

Ordering of stock/consumables as required for the Practice and as part of the nursing team.

Ordering of dressings as required as part of the nursing team.

Daily checks required for infection control/cold chain policy etc
