Job summary
We are seeking a compassionate, organised, and motivated Part-time Practice Nurse to join our friendly primary care team on a Monday and Friday (with potential additionally for Wednesday). This is an excellent opportunity for a registered nurse who is passionate about delivering high-quality patient care within a supportive GP practice environment. The successful candidate will play a key role in providing preventative healthcare, chronic disease management, health promotion, and routine nursing services to our diverse patient population.
Working collaboratively with GPs, healthcare assistants, and the wider multidisciplinary team, the Practice Nurse will help ensure safe, effective, and patient-centred care while contributing to the smooth day-to-day running of the practice. We welcome applications from experienced Practice Nurses as well as nurses looking to transition into primary care with appropriate support and development opportunities available.
Main duties of the job
Deliver high-quality nursing care within the GP practice setting. Carry out routine clinical procedures including vaccinations, immunisations, cervical screening, wound care, and health checks. Support the management of patients with long-term conditions such as diabetes, asthma, and hypertension. Provide health promotion advice and support patients in maintaining healthy lifestyles. Undertake patient assessments, treatment, and follow-up care in line with clinical protocols. Maintain accurate and up-to-date patient records using clinical systems. Work collaboratively with GPs and the wider multidisciplinary team to ensure effective patient care. Contribute to infection prevention, safeguarding, and clinical governance activities.
Participate in training, practice meetings, and continuous professional development.Home visits to deliver chronic disease management and immunisations for patients who are housebound/residing in care/nursing homes;Full access to a vehicle for work purposes and possession of appropriate business-use motor insurance is essential.

Assist and perform routine tasks related to patient care as directed by the GP Partners
To deliver Primarily Chronic Disease Management in Diabetes, Asthma, COPD (including CHD, Hypertension, CKD, Rheumatology etc). Implement and evaluate individual treatment plans for patients with a long term condition.
Routine immunisations Childhood Immunisations: Understanding requirement for PGD and PSD.
Travel immunisations
Cytology Swabs
Wound care minor injuries
Urinalysis
Venepuncture
Ear Syringing
Shared Care Prescribing Monitoring/Annual Review
Primary Prevention Clinics
Performance of ECG
There will be a requirement to undertake home visits (Chronic Disease Management, Routine Immunisations, Venepuncture): The Practice Nurse will be expected to have a full driving licence and transport available.
Chaperoning and assisting patients where appropriate who are being examined by another clinician
Assisting GP Partners with minor surgery
Requesting pathology tests, for example urine culture, swabs
Other tasks will include:
Maintaining and cleaning equipment used by the nurses and GP Partners
Maintaining Nurses rooms, stocking and rotating items as required
Maintain general tidiness and cleanliness of nurses and treatment rooms
Participation in administrative systems in the practice
At all times there will be a need to maintain accurate records
Attend and participate in any Practice meetings when required
Any other delegated duties appropriate to the post
Provide occasional cover for annual leave and sickness
Person Specification
Qualifications Essential
Registered Nurse qualification with valid NMC Registration. Driving licence holder with sole use of vehicle for business usage.
Desirable
Accredited Training in Chronic Disease Management Clinical Skills Cervical Cytology, Childhood immunisations/vaccination, ear care Wound Care Management
Experience Essential
Experience of working within a nursing team Experience of dealing with vulnerable patients Ability to work core hours and extended hours as per PCN DES Flexibility for cover
Desirable
Experience of working in primary care Experience of working in a GP practice
