About this role

Job summary

We are seeking an enthusiastic individual to take on the role of Practice Nurse. The role is to ensure the delivery of an effective Practice Nurse service to the practice population, delivering care appropriately according to practice policy, protocols, guidelines and procedures.

Main duties of the job

To carry out a wide range of treatment room duties e.g: venepuncture, new registration checks, blood pressure checks, ECGs, vaccinations and immunisations To run Chronic Disease Clinics such as CHD, Diabetes and Respiratory. Offer appropriate lifestyle advice to patients. To provide support to the nursing team to ensure smooth running of the nursing service.

Practice Population Circa 11,000 Patients. Friendly and passionate team of 5 GPs and 3 Advanced Nurse Practitioners. Excellent management and well-established support team to ensure the practice is well organised and structured. Friendly, supportive atmosphere. Practice uses SystemOne practice software with Ardens templates Dedicated pharmacy team who handles all medication related queries and repeat prescriptions Dedicated in-house PCN staff including CPs, ACPs, Mental Health Workers, Care Coordinators and Health and Well-Being Coaches Dedicated Community and Home Visiting Teams Large, modern, purpose-built surgery with excellent facilities.

Job description Job responsibilities

Clinical Practice:

Assess, plan, provide and evaluate treatment programmes including, but not limited to:Manage and treat patients with long term conditions in line with national / local policies and practice needs with the inclusion of Home VisitsReview medication for therapeutic effectiveness in accordance with evidence based practice and national / practice protocols, and work with patients to support adherence to prescribed treatmentsSupport and manage health needs of women presenting for Family Planning and Cervical Cytology ConsultationsImplement and participate in Vaccination and Immunisation Programmes for both Adults and Children

Health & Safety

The post-holder will implement and lead on a full range of promotion and management their own and others health and safety and infection control as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include (but will not be limited to):

Using personal security systems within the workplace according to practice guidelines Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines Responsible for the correct and safe management of the specimens process, including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements Responsible for hand hygiene across the practice Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process Safe management of sharps procedures including training, use, storage and disposal Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile, and safe way, free from hazards. Initiation of remedial / corrective action where needed or escalation to responsible management Actively identifying, reporting, and correction of health and safety hazards and infection hazards immediately when recognised Keeping own work areas and general / patient areas generally clean, sterile, identifying issues and hazards / risks in relation to other work areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business in consultation (where appropriate) with other sector managers Undertaking periodic infection control training Spillage control procedures, management and training Decontamination control procedures, management and training, and equipment maintenance Maintenance of sterile environments Demonstrate due regard for safeguarding and promoting the welfare of children.

Person Specification

Qualifications Essential

Qualified Nurse. Experience of GP Practice Nursing. Full UK Driving Licence.

Desirable

Prescriber Diploma in respiratory
