About this role

Job summary

To provide assessment whenever service users present in crisis To triage referrals and undertake crisis assessments, and routine interactions (where required). To reduce the likelihood of duplicate assessments for individuals. To work closely with referrers providing expert guidance and support with a view to increasing the threshold of acuity of mental ill health cases managed in primary care (in line with a stepped model of care). To provide care and support in a comfortable and relaxed non clinical environment To harness the concept of alternative provision to A& E attendances in order to relieve pressure in General Hospitals and to advise service users on opportunities for providing care closer to home as a viable alternative to hospital admission. To ensure individuals are directed to the primary or secondary care service which is best suited to meet their needs. To liaise with the key services or the out-of-hours Social Services Emergency duty team, in order to arrange Mental Health Act assessments where needed. To deliver evidence-based interventions for a time- limited period, in order to reduce the likelihood of the crisis episode reoccurring and to increase resilience. To develop comprehensive care packages for service users who have received a face-to-face assessment and signpost them as required.

Main duties of the job

Embody the visions and values of Surrey & Borders Partnership Foundation NHS Trust Develop, implement and evaluate evidence based programmes of care; set standards of care and liaise with other agencies, particularly in relation to people discharged under the Care Programme Approach Participate in the multi-disciplinary assessment of individuals with mental health problems or vulnerabilities living in the community, by providing a community mental health perspective and expertise Ensure that a risk assessment is completed for service users, especially if there is a history of, or a potential for risk Ensure that a carers assessment is offered to carers of people who use services, in line with Standard 6 of the National Service Framework Be responsible for the full assessment of the psychiatric/psychosocial needs of those patients who have been referred to the service; including the comprehensive assessment of risks to self and others Be responsible for the development, implementation and evaluation of robust discharge plans based on a clear assessment and formulation of the person's needs; which may include urgent referral to secondary mental health services, social and voluntary services.
