We are looking for a Registered Nurse (Adult) to deliver the full spectrum of practice nursing services to our patient population. You will play a key role in chronic disease management, immunisation and vaccination programmes, treatment room services, and health promotion and screening initiatives. You'll be part of a supportive, forward thinking team and will also work closely with our Primary Care Network to help deliver services at scale alongside a wider MDT of Care Coordinators, Pharmacists, Physiotherapists and a Social Prescriber

Chronic disease management - supporting patients with longterm conditions through evidencebased care Treatment room services - wound care, dressings, ECGs, ear care, and other routine procedures Health screening - delivering and supporting national screening programmes Immunisation and vaccination - childhood, adult, seasonal, and travel vaccinations

You will be joining an established and friendly rural Lincolnshire practice team consisting of 3 GPs, a GP Registrar, Practice Nurse and Phlebotomist

11. Job Summary
The Practice Nurse provides safe, evidence based nursing care to the practice population. Working autonomously and as part of a multidisciplinary team, the postholder delivers chronic disease management, immunisations, screening, wound care, health promotion, and contributes to population health improvement.
2. Key Responsibilities
Clinical Care
Chronic disease management - diabetes, asthma, COPD, hypertension, frailty reviews.
Immunisations & vaccinations - childhood, adult, travel, seasonal campaigns.
Cervical cytology - screening in line with national guidance.
Wound care and treatment room services - dressings, compression therapy, postoperative care.
Clinical procedures - ECGs, venepuncture, spirometry, BP monitoring, NHS Health Checks. Assisting the GP with minor surgery
Health promotion - lifestyle advice, smoking cessation, weight management.
Long-term condition recall - proactive management using practice protocols.
Professional Responsibilities
Maintain accurate clinical records.
Work within the NMC Code and scope of professional practice.
Participate in audits, QI projects, and clinical governance.
Support infection prevention and control (IPC) standards.
Contribute to safeguarding of children and adults.
Engage in CPD and revalidation activities.
Team & Service Development
Work collaboratively with GPs, ANPs, HCAs, pharmacists, wider MDT and admin teams.
Assist in developing and reviewing clinical protocols.
Support new staff and students.
Contribute to population health initiatives and reducing health inequalities.
Person Specification
Experience Essential
Experience delivering nursing care within scope of practice. Immunisations, wound care, and basic clinical procedures. Working with clinical guidelines and protocols. Use of clinical IT systems (SystmOne).
Desirable
Experience in General Practice or Community Nursing. Chronic disease management clinics. Travel health. Quality improvement or audit work.
Qualifications Essential
Registered General Nurse (RN Level 1) with current NMC registration. Evidence of ongoing CPD.
Desirable
Cervical cytology training. Immunisation training. Chronic disease management modules (e.g., diabetes, respiratory). Family planning qualification. Practice Nurse qualification Independent prescribing (V300)

Knowledge and Skills Essential

Desirable
Knowledge of population health management. Understanding of QOF and enhanced services.
