Job summary
We are looking for an additional Practice Nurse to join our a friendly practice. Based in Central Middlesbrough we have a population of 7000 patients. You will be working as part of a team alongside another two Practice Nurses and a HCA, all of whom have been with the practice a long time. Our practice has won the Healthwatch South Tees STAR Special Recognition Award in 2023 and we were also shortlisted for the Service Award. In 2024 the Practice was shortlisted for the PMA Practice Award and one member of our reception team won the PMA Lifetime Achievement Award beating out consultants and GPs from around the country. The practice along with eight individual members of the team all won Healthwatch STAR awards in 2025. The practice is high performing with QOF and LD reviews as well as one of the highest performing in the country for Health Heart Checks and has been rated highly in the Daily Mirror polls for appointments in the last 3 years running across the country, in the top 200 out of 6,500 practices.
Due to the location of the practice we are classed as a Deep End Practice and are involved in schemes around making the life of our patients better, this is something we would like you to be involved in.
Main duties of the job
Each day starts with time out for daily audits, this allows the Practice Nurse or HCA to check the rooms, equipment and consumables to ensure everything is in place. Audit time is also given at the end of each clinic. Appointment times vary depending on the nature of the appointment. We do not have set clinics like a full session of childhood immunisations or chronic disease management, we do have set times from 8am - 12noon for a morning and 1pm - end of shift time, these include the daily audit times, we find the diversity keeps the sessions varied and fresh so you will never become bored and hopefully will feel excited to come to work daily.
The Nursing team works closely with the rest of the practice and we are proud to say we are all part of the team that works together. The staff arrange regular team building for anyone to attend and we have done bowling, escape rooms, meals out and even Bingo.
Stock checks are done monthly, we also do weekly checks on clinical rooms for stock and infection control. All audits are documented and time is allowed for these.
We would prefer if you have experience in various areas of General Practice including chronic disease management, childhood immunisations, wound management and you will be expected to take bloods and perform ecg's. Do not let it put you off if you do not have everything on the desired list as we will provide training to the right candidate. The other two Practice Nurses can already do everything between them.
About us
The position is to work at the practice 3 days per week, every Monday, Tuesday, and Friday. As stated in the opening we are a friendly Central Middlesbrough Practice and heavily involved with our PCN, Local Medical Council and Deep End Network.
We are a forward thinking practice and our team are fantastic, they all work together and help each other. The staff are appreciated and we have a flexible approach to work so if anyone needs some time off, say to attend an event, if we can with enough notice we will try to move clinics around where possible to save having to use annual leave.
The staff are award winning and very friendly. Our appointment system is such that some days we even have spare appointments at the end of the day for the GPs. We can access a lot of other services for patients and our PCN runs it's own Extended Access so we have availability to not only utilise their appointments for GP & Nurses but our staff have the opportunity to work in the service if they want overtime.

To respond to the general nursing requirements of practice patients.
Be responsible for meeting the nursing care needs of patients identified by the nurse, or where care has been transferred to the nurse by the GP
In accordance with the NMC formulate individual care plans and evaluate outcomes at appropriate levels according to need.
To evaluate the outcome of that care, making changes to the care plan for individual patients where appropriate.
To demonstrate the rationale behind the various methods chosen to meet care needs of patients, to evaluate those methods and change them as necessary.
Review and plan care for patients with long term conditions such as- diabetes, asthma, COPD.
Undertake cytology screening (on completion of training) with full knowledge of the national and local screening programmes and providing advice and follow up when appropriate.
To promote healthy lifestyle including advice on diet and exercise, alcohol and to be aware of family history.
Provide full and accurate documentation of the nursing assessment of need and delivery of care.
Liaise with other members of the primary health care team and other agencies to ensure appropriate care is provided for the practice population.
