About this role

Job summary

We are looking for an experienced Practice Nurse who can provide care to our patients, working alongside our experienced nursing team. We are a busy practice with a list size of approximately 13,500 but we also like to enjoy our days at work and therefore a sense of fun is required ! We are looking for a nurse who is already up and running with the management of long term conditions and who maybe willing to expand their knowledge of further training, if needed. Please note interviews will be taking place on Tuesday 9th June 2026.

Main duties of the job

The successful applicant will be responsible for the oversight of our patients requiring long term condition reviews in the major disease areas; diabetes, respiratory, CHD, and frailty along with childhood immunisations, smears and other vaccinations. We can provide a friendly and supporting working environment, joining our existing team of 4 Practice Nurses, 3 HCA's, and a team of GP's and ANP's. Not to forget our excellent supportive admin team. Access to the NHS Pension and a private Simply Health plan is available, along with access to further training should this be required.

About us

Neasham Road Surgery is a busy practice with a list size of approximately 13,500. We have a good CQC rating and use systmOne clinical system. We have an excellent team of clinical staff; 3 GP Partners, 3 Salaried GP's and 4 ANP's, all providing healthcare to our large population. In addition we have a hard working but fun nursing team who will be working alongside you provide advice, support and friendly chat. Our admin team are also there to help you and we encourage working as one big team. You will have your own appointment list with appropriate times given to the task needed along with some admin time each day to follow up. We provide regular nurse meetings run by our Nurse Lead (ANP) and also encourage training and further education which will help both yourself and the Practice. We like to let our hair down occasionally and have two set evenings each year with all staff invited.

Job description Job responsibilities

To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team.

Job Responsibilities:

Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs Chronic disease management in at least two disease areas (respiratory, diabetes, family planning, CHD, hypertension) Urinalysis ECGs Venepuncture Blood pressure measurements Routine immunisations Travel vaccinations Requesting basic pathology tests, for example urine culture, swabs using ICE Following agreed clinical protocols with referral to senior nurses or GPs as appropriate Infection Control Other tasks including: Chaperoning and assisting patients who are being examined by another clinician. Undertake housekeeping duties including, general tidiness and cleanliness of nurses and treatment rooms Participation in administrative systems in the Practice A duty to advise senior nurses of potential problems or errors within the range of assigned tasks Maintain accurate records Attend and participate in any Practice meetings when required. Any other delegated duties appropriate to the post.

Health & Safety:

The post-holder will implement and lead on a full range of promotion and management their own and others health and safety and infection control as defined in the practice Health & Safety Policy, the practice Health & Safety Manual, and the practice Infection Control policy and published procedures. This will include (but will not be limited to):

Using personal security systems within the workplace according to Practice guidelines Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines Responsible for the correct and safe management of the specimens process including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements Management and maintenance of Personal Protective Equipment (PPE) for the practice including provision, ordering, availability and ongoing correct usage by staff Responsible for hand hygiene across the practice Ownership of infection control and clinically based patient care protocols, and implementation of those protocols across the practice Active observation of current working practices across the practice in relation to infection control, cleanliness and related activities, ensuring that procedures are followed and weaknesses / training needs are identified, escalating issues as appropriate Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process Making effective use of training to update knowledge and skills, and initiate and manage the training of others across the full range of infection control and patient processes Monitoring practice facilities and equipment in relation to infection control, ensuring that provision of hand cleansing facilities, wipes etc are sufficient to ensure a good clinical working environment. Lack of facilities to be escalated as appropriate. Safe management of sharps procedures including training, use, storage and disposal Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile, and safe way, free from hazards. Initiation of remedial / corrective action where needed or escalation to responsible management Actively identifying, reporting, and correction of health and safety hazards and infection hazards immediately when recognised Keeping own work areas and general / patient areas generally clean, sterile, identifying issues and hazards / risks in relation to other work areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business in consultation (where appropriate) with other sector managers Undertaking periodic infection control training (minimum twice annually) Routine management of own team / team areas, and maintenance of work space standards Waste management including collection, handling, segregation, container management, storage and collection Spillage control procedures, management and training Decontamination control procedures, management and training, and equipment maintenance Maintenance of sterile environments

Person Specification

Experience Essential

Minimum of 2 years post registration Experience in primary care/GP surgery Experience in chronic disease areas Experience of cervical cytology Experience of childhood immunisations

SystmOne Computer experience/knowledge Willingness to train further Experience or qualification in infection control

Qualifications Essential

Registered Nurse qualification Phlebotomy qualification Chronic disease qualification - certificate in any areas (respiratory, CHD, diabetes etc)
