About this role

Job summary

Are you looking to join a highly skilled multidisciplinary team who are passionate about promoting formulation driven care? An exciting opportunity has arisen to enhance the care of young people with complex emotional needs as they make the transition into Adult Mental Health Services.

The successful applicant will work to strengthen links with relevant teams under Children, Young People and Families Health Services, as well as with other partner agencies supporting young people in the Cardiff and Vale area, with a view to ensuring that their mental health care remains cohesive, effective and formulation driven during a period which is typically associated with significant uncertainty and change for the young person and their support network.

The post will involve assisting professionals in understanding and supporting complex, high-risk individuals. This includes supporting teams to enhance their formulation of and response to behaviours that can challenge relationships and systems, as well as to recognise when a pattern of interaction between a service user and their care network may be inadvertently re-enacting the service user's previous experiences of being cared for.

The Cynnwys team are a warm, supportive team where curiosity is encouraged and collaborative working is valued. Supervision and CPD are viewed as essential to the role and the successful applicant will receive ongoing support to develop the highly specialist skills required for the post.

Main duties of the job

To hold a small caseload of high-risk young people with complex emotional needs who may go on to attract the diagnosis of Personality Disorder and who require transition to secondary mental health services.

To work in partnership with young people, carers and involved professionals to promote a person-centred, coproduced approach to the care and treatment that is offered to individuals who, at times of intense distress, may cause themselves serious harm.

To assist colleagues within services for Children and Young People's Emotional Wellbeing and Mental Health and within Adult Mental Health, in the development of a shared formulation and plan for a young person's ongoing care as they transition between different services.

To provide a leadership and consultancy resource, providing education and support to multidisciplinary colleagues in the effective care and treatment of individuals with complex emotional needs.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Person Specification

Qualifications Essential

Health related professional qualification (RMN/Psychology/Social Work/OT) Masters degree or equivalent experience Maintains current registration with the NMC, HCPC or other relevant professional registration

Desirable

Formal training in supervision or reflective practice Prior therapy qualification or CPD certificate (e.g. in CBT, DBT, MBT)

Experience Essential

Significant experience of working with individuals who present with multiple needs and risks. Prior experience in working with young people and families within a CAMHS service. Prior experience of delivering formulation led, psychologically informed interventions. Prior experience in assessing and managing suicidality or self-injury. Experience in providing supervision / consultation to other clinicians. Experience of effective multi-agency working. Prior experience of delivering training to others. Experience of successful change management and dealing with conflict. Experience of participating in the planning and development of services.

Desirable

Prior experience of trauma informed ways of working Experience of working with families, drawing on a systemic knowledge and practice base. Experienced in speaking & lecturing in public & professional forums.

Skills and Competencies Essential

High level communication and interpersonal skills to convey and receive complex, highly technical, sensitive information effectively at both a written and oral level. Ability to engage others in collaborative change processes. High level of presentation and report writing skills for clinical and academic content in a range of contexts. Ability to make clinical and service-related judgements involving complex facts requiring analysis, interpretation and comparison of a range of options. Ability to critically evaluate research evidence relating to clinical effectiveness and service development.

Desirable

Experience in presenting papers in local and regional professional forums. Ability to speak Welsh/other non-English languages and a willingness to use this skill in the work context.

Special Knowledge Essential

Specialist skills in working with complex mental health problems. Familiarity with key legislation in working with young people. Skilled and experienced in providing clinical supervision to a range of clinicians. Ability to independently navigate patient clinical records to pull out key information to support the development of a formulation.

Desirable

Good understanding of and familiarity with transitions literature.

Knowledge Essential

Up to date knowledge of legislation, policy and professional issues pertinent to the fields of Personality Disorder, trauma and self-injury. Professional knowledge acquired through degree/diploma supplemented by specialist training, experience, and short courses.

Personal Qualities Essential

Strategic thinker. Able to lead complex communication processes where there are opposing views and significant barriers to acceptance and understanding. Able to work consistently and therapeutically with frequent exposure to highly emotional, distressing and sensitive clinical circumstances. Practices equal opportunities and respect for others.
