Job summary
OverviewWe are seeking a highly organised and proactive Practice nurse to be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients. Act as the Infection Prevention Control Lead.
Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.
The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health promotion, well women (Cervical Screening ) and well man clinics, Immunisation, Emergency drug check, ordering of vaccination, Travel clinics as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
Houghton Regis Medical Centre must ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.

Delivering direct patient care, including health assessments, vaccinations, and chronic disease management Assisting GPs with examinations, procedures, and diagnostics Providing health education and advice tailored to individual patient needs Managing patient records accurately and maintaining confidentiality at all times Supporting the practice in meeting clinical governance standards and protocols Administering treatments and medications as prescribed by the GP Participating in practice meetings and continuous professional development activitiesQualifications
Registered General Nurse (RGN) with valid registration with the Nursing and Midwifery Council (NMC) Minimum 2 years post registration. Proven experience working as a Practice Nurse or in a similar primary care environment Excellent patient care skills with a compassionate approach Ability to work effectively within a multidisciplinary team Organised, reliable, and able to manage a varied workload efficiently Commitment to ongoing professional development and maintaining high standards of practice

Qualifications Essential
An experienced Practice Nurse who can perform daily Nurse clinics, dressings, childhood Immunisation, all Injections, travel Clinic, Cervical Smears, ECG and all Nursing duties including Long term Conditions reviews
Desirable
Experience as Practice Nurse in a GP surgery.
