Job summary
We are seeking a compassionate and motivated Practice Nurse to join our friendly GP surgery team. The successful candidate will be based at our Fairfield site, working closely alongside our existing nursing team at our Kingston Hill site to provide high-quality patient care across both locations. Duties will include chronic disease management, health promotion, immunisations, cervical screening, wound care, and supporting GP clinics within a busy primary care environment. Excellent communication skills, teamwork, and a commitment to delivering safe, patient-centred care are essential.
Main duties of the job
- Working alongside doctors and other healthcare professionals to deliver care
- Providing a safe, accessible and high-quality treatment for the patient population
- Assessing and managing patients using an evidence-based approach
- Being responsible for various clinical areas including health promotion and prevention, chronic disease management and screening
