St Johns Hospice has a busy and friendly community-based Hospice at Home service, working closely with wider community healthcare professionals to support patients who wish to remain at home. Our team is part of the Care Coordination Service, helping to ensure patients receive responsive, compassionate, and high-quality end-of-life care.

About the job
We are looking to recruit an enthusiastic and experienced Registered Nurse to join our Hospice at Home team in a leadership role.
This role combines both clinical and management responsibilities, supporting the delivery and development of the Hospice at Home service. You will support the coordination, leadership, and supervision of Senior Healthcare Assistants and the wider team.
The service works in partnership with community healthcare professionals to deliver high-quality palliative and end-of-life care. As part of the Care Coordination Service, the team also contributes to an advice line, and you will be expected to support this and occasionally take calls.
There may also be occasions where you will accompany Healthcare Assistants on patient visits to provide additional clinical support and guidance.

About you
You will be a motivated and compassionate Registered Nurse with experience in palliative care or community nursing (or a related field). You will be confident working both independently and as part of a team, with the ability to balance clinical responsibilities alongside leadership and coordination duties.
You will have experience supporting or leading teams, with strong communication skills and the ability to adapt to a dynamic community environment. A passion for delivering high-quality, patient-centred care at home is essential.

Registered Nurse (NMC registered) Experience in palliative care, community nursing, or a related clinical setting Demonstrable clinical leadership or team coordination experience Excellent communication and interpersonal skills Ability to support and supervise staff in a community setting Experience with managing change and working in a responsive service Ability to contribute to an advice line and care coordination service
Desirable
Experience working within a Hospice at Home or community palliative care service Experience in staff mentoring, teaching, or clinical development Knowledge of end-of-life care pathways and community services
