Sue Ryder offers a range of hospice and community nursing services in Berkshire West and South Oxfordshire, including Reading, Newbury, Wokingham, Wallingford and Henley, and many more local communities in between.

About You

We are looking for a compassionate and organised Registered Nurse with current NMC registration and experience supporting patients with palliative care and or long-term conditions. You will be confident working in a fast-paced and occasionally challenging environment, using your clinical leadership skills and initiative to coordinate admissions and discharges safely and effectively.

The successful candidate will have excellent communication skills, be able to support patients and families through sensitive conversations, and work collaboratively with multidisciplinary teams. You will also be confident mentoring others within a clinical setting, managing complex situations, and using IT systems effectively. Experience in coordinating end-of-life care and supporting patients to achieve their preferred place of care is highly desirable.

About the role

We have an exciting opportunity for an Admissions and Discharge Coordinator to support our In-Patient Unit . This role is ideal for a Registered Healthcare Professional with experience or interest in coordinating referrals, admissions, and timely patient discharges within palliative care.

You will act as the first point of contact for hospice referrals, carrying out initial clinical triage and coordinating safe, effective discharge planning for patients returning home or moving to an appropriate care setting. The role involves working closely with internal and external multidisciplinary teams to ensure high-quality, seamless patient care, while also supporting junior staff and volunteers within the service.

The successful candidate will work in line with professional standards, Sue Ryder policies and procedures, and participate in the senior nurse on-call rota.
