NCIC is expanding and investing in its Frailty services to integrate between community and secondary care. North Cumbria has an ageing population and is seeing more older people living with frailty in our emergency departments and on our wards. We know that a hospital stay makes it less likely that an elderly person can return home and live independently so we have introduced a new Frailty Hub to provide a same day service.

After a successful pilot, the service is now being permanently funded in both Whitehaven and Carlisle; therefore we are looking for dynamic and motivated team members to join our ambitious, multi-disciplinary team. We want to hear from people who are passionate about our vision to provide more care closer to home and enhance the care we provide to older people living with frailty.

Main duties of the job

We are looking for posts in both West Cumberland Hospital (WCH) and Cumberland infirmary Carlisle (base sites), however some cross cover may be required. The successful candidates will play an important role in embedding a Same Day Care approach to frailty. As a developing service, there are currently no substantive medical consultant posts within the frailty pathway, and the successful candidate will contribute to shaping clinical standards, pathways, and ways of working, in line with national guidance and local priorities.

The Same Day Care Frailty Service is intended to support timely assessment and management of patients living with frailty, with a focus on ensuring patients receive the right care, in the right place, at the right time. The service will work closely with the Emergency Department, ambulance services, and primary care to provide early senior clinical input, support decision-making, and where appropriate, enable same-day management and discharge with appropriate support.

The development of this service is an important part of the Trust's response to ongoing system pressures, including increasing demand, Emergency Department crowding, and delays in patient flow. The post holder will support efforts to reduce avoidable admissions, improve patient experience, and contribute to more effective use of hospital capacity, while maintaining a strong focus on patient safety and quality of care.

Job description Job responsibilities

The appointee will, together with existing colleagues be responsible for the provision of a comprehensive, efficient, and cost-effective clinical frailty service to the Trust and other users of the service.

The successful candidate will be expected to:

Work with colleagues to develop an acute frailty pathway and further develop frailty services in partnership with the Integrated Care Communities.

Provide teaching for Junior Doctors, Medical Students and other members of the multidisciplinary team

Take responsibility for work in the Frailty hub based at WCH or CIC.

Share with Consultant colleagues the role of educational or clinical supervisor to junior doctors in training including induction, provision of supportive formal appraisals, supervision of learning portfolios, and career advice as appropriate.

Undertake personal development and mandatory training

Participate in clinical meetings and post graduate activities in the Trust.

Participate in Clinical trials

Work closely with other specialist teams

Work with managers and professional colleagues to ensure the efficient running of services

Undertake administrative duties associated with the care of their patients and the running of clinical areas and also as required by the Clinical Lead, Clinical Director, Executive Medical Director and Chief Executive.

Assist in the preparation of business plans.

Support and participate in the Booked Appointment Systems

Take an interest in research/teaching with participation in projects and courses

Participate in Consultant Appraisal and Job Plan Review on an annual basis and the revalidating process when necessary. The Trust will support revalidation

Person Specification

Qualifications Essential

Valid Full Registration with the General Medical Council Entry on the General Medical Council (GMC) Specialist Register via one of the following: a) Certificate of Completion of Training (CCT) (the proposed CCT date must be within 6 months of the interview) b) Certificate of Eligibility for Specialist Registration (CESR) c) European Community Rights. Membership of the Royal College Physicians or equivalent

Postgraduate qualification

Experience Essential

High level of clinical experience and competence in Geriatric medicine Completed Higher Specialist Training (CCT) in Geriatrics or evidence of equivalent competency. Excellent communication skills with patients, colleagues, managers and other staff Ability to take full and independent responsibility for clinical care of patients

Desirable

A commitment to, and experience of teaching An interest in developing links with Acute Medicine or Primary Care
