Main duties of the job

Job summary
The Practice Nurse will provide high-quality nursing care within a primary care setting, supporting patients across a range of health needs. The role involves delivering nurse-led clinics, managing long-term conditions, and providing preventative care and health promotion.
You will work both independently and as part of a multidisciplinary team to ensure safe, effective, and patient-centred care.
Key responsibilities
Clinical duties
Management of chronic diseases including asthma, diabetes, and hypertension
Cervical screening
Immunisations and vaccinations (adult and childhood where applicable)
Health promotion and preventative care
Nurse-led clinics
Patient care
Provide advice, support, and education to patients and carers
Promote self-management of long-term conditions
Deliver care in a safe, respectful, and patient-centred manner
Team working
Work collaboratively with GPs, nurses, HCAs, and administrative staff
Contribute to effective running of clinical services
Participate in team meetings and clinical discussions as required
Professional responsibilities
Maintain accurate clinical records
Work within professional guidelines and practice policies
Maintain and develop clinical skills through ongoing CPD
Person Specification
Qualifications Essential
Registered Nurse with current NMC registration Significant experience working as a Practice Nurse or in a primary care setting Competent in cervical cytology Competent in immunisation and vaccination administration Proven experience managing long-term conditions (including asthma, diabetes, and hypertension) Experience running nurse-led clinics in primary care Strong clinical assessment, communication, and interpersonal skills Ability to work independently and as part of a multidisciplinary team Commitment to high standards of patient care and ongoing professional development
Desirable
Experience in phlebotomy Additional training in immunisation programmes beyond core requirements Diabetes specialist training or enhanced experience in diabetes management Experience in chronic disease management clinics Experience with audit, QOF, or quality improvement work in primary care Additional CPD in areas such as minor illness or advanced clinical skills
