About this role

Job summary

An exciting opportunity has arisen to join our nursing team for an additional Practice Nurse to join our friendly practice working 16 hours per week.

The ideal candidate will be a Practice Nurse with general practice experience, who will work alongside our existing nursing team.

We are a busy, high achieving practice with just over 12,000 patients located in Hartlepool.

About us

Our clinical team currently consists of 3 partners and 2 Salaried GP, 1 Pharmacist, 1 Nurse Manager/ANP, 1 ANP, 2 Nurse Practitioners, 2 Practice Nurses, 2 HCA's & 1 ARRS GPA. Our practice also has a large team of PCN staff working alongside consisting of Salaried GP's, Pharmacists, Social Prescribing Link Workers, Care Co-ordinators, Health and wellbeing Coaches, FCP's and Mental Health Practitioners. The above teams are supported by our large team of admin and reception staff.

All of our teams work exceptionally well together and support each other especially through difficult times and situations.

Professional:

Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including; vaccinations and medicines, safe sex, food hygiene, sun protection. Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions. Running well person clinics and health promotion programmes by providing a holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise, facilitating change. Ability to document Consultations according to NMC guidelines Ensure awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse. Perform a holistic assessment of patients attending for ear irrigation. Perform a holistic assessment of patients attending for cervical cytology smear tests. Perform venepuncture according to local guidelines Provide a holistic approach to wound management and implement wound care in line with current evidence based guidelines. Suture /staple/clip removal Administer Zoladex Injections Ability to recognize and manage anaphylaxis according to current UK guidelines. Ability to perform Cardio-pulmonary resuscitation according to current UK guidelines. Assist the doctor with insertion and removal of coils & implants. Ability to obtain and document informed consent (either verbal or written). Ensure infection control guidelines are maintained. Ability to monitor and manage maintenance of stock and equipment to include refrigeration and emergency equipment. Ability to demonstrate through clinical practice and documentation, an awareness and working knowledge of local/national protocols. Ability to demonstrate the delivery of skilled nursing care which is current and Evidence Based. Demonstrate articulate factual documentation within patient case notes. Confidentially of information gained at work must be preserved at all times. Undertake Home visits to perform clinical duties within professional parameters Perform minor ops to treat warts and verrucas following satisfactory completion of competencies as a result of a robust training programme Participation in achieving Quality Outcomes Framework (QOF) targets Demonstrate effective time management skills. Contribute to the assessment of service needs. Assist patients to identify their health needs. Contribute a nursing perspective to the practice development plan. Manage and organise individual patient consultations. Aware of identification and reporting procedures related to professional standards. To attend staff meetings/clinical supervisions.
