Job summary
An exciting opportunity has arisen to join an experienced team of complex discharge coordinators across Greater Manchester's inpatient Neurological Rehabilitation services.
These services include hyper-acute neuro-rehabilitation beds at The Nothern Care Alliance Foundation Trust, as well as post-acute beds at Trafford INRU and Devenshire Centre and Floyd Unit
You will be based at Trafford INRU. Successful applicants will need a full driving license and be expected to travel to the other units to provide cross cover as required.
Main duties of the job
o Timely, safe, and well-coordinated discharges for all patients within the GM Neuro-Rehabilitation Inpatient Serviceo Improved patient and family experience of the discharge processo Reduced delays and improved patient flow across the serviceo Strong, effective working relationships with internal teams and external partnerso High-quality data, audit outcomes, and service improvement initiativeso Consistent application of legal frameworks including MCA and DoLSo A flexible, responsive, and patient-centred discharge service

Qualifications & Professional Registration Essential
Registered Nurse with current NMC registration (Part 1 Adult / General Level 1 or 2) Or AHP or Social Work England registration or equivalent professional registration Degree Level studies or an equivalent portfolio of evidence
Desirable
Post basic qualification in specialty
Knowledge, Training & Experience Essential
Evidence of professional/clinical knowledge acquired through CPD and experience Knowledge of Discharge planning policies, legislation and guidance and their implementation Demonstrates involvement and leadership in teaching and mentorship Demonstrates evidence of involvement in development of protocols and clinical audit Demonstrates ability to work within multi-agency / professional setting Demonstrates leadership qualities Able to deputise in the absence of the Team leader Able to analyse and make decisions in relation to patient risk and care in complex cases Ability to travel to different geographical locations across the Trust footprint in line with service needs
Desirable
Knowledge of community services including Intermediate Care, Community Nursing and Social Care Commissioning Full UK driving license with willingness to use own car for work purposes
