About this role

Job summary

An exciting opportunity has arisen for a Practice Nurse to join our team at Fell Cottage Surgery. Our Practice is located in the Low Fell area of Gateshead, providing care to a practice list size of approximately 9,000 patients.

We are looking for an experienced Practice Nurse who will lead on Long Term Condition management including diabetes. The successful candidate will also play a key role in childhood immunisations, cervical screening and health promotion within our busy GP Practice.

We have a strong team ethos at Fell Cottage Surgery, ensuring our patients are at the centre of everything we do .

Interviews will be held between 9th and 11th of June 2026

Main duties of the job

The Practice Nurse will deliver high quality, evidence based practice nursing services as an autonomous practitioner, with a lead role in long term conditions, working within the multidisciplinary team to meet the needs of the practice population.

The successful candidate will work with our dedicated nursing team delivering childhood immunisations, cytology, vaccinations, health promotion, as well as actively supporting the practice team in delivering clinical policy and procedures.

Fell Cottage Surgery will ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.

Fell Cottage surgery is located in the heart of Low Fell in Gateshead, our surgery is a listed building which was once a residential property. We are lucky to led by two GP partners, five salaried GPs and one Advanced Nurse Practitioner, supported by a great nursing team consisting of two Practice Nurses and two Health Care Assistants

We are a training practice supporting GP trainees, Foundation Doctors and Medical Students. Our Admin team of 11 staff are supported by our newly appointed Administration Manager. Our surgery benefits from having our own Patient Care Coordinators who are led by our Quality Lead working directly with our patients

We also work alongside our extended team of Additional Role Staff which includes Pharmacists, Counsellors, Mental Health Practitioners, Social Prescribers, Physiotherapists, Midwife and Health Visitors. We have a family feel whilst striving to deliver the best healthcare to our patient population

Primary key responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

Provide high quality, evidence-based care for patients with long term conditions such as diabetes, asthma, COPD, hypertension and cardiovascular disease.Support the management of long term conditions through routine monitoring and reviews.To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practiceTo develop, implement and embed health promotion and well-being programmesTo implement and evaluate individual treatment plans for chronic disease patients that promote health and well-beingAbility to interpret long term condition bloodsTo act as support to the Infection Prevention Control (IPC) lead and to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the practice managerTo support the production of the IPC annual statement in conjunction with the IPC leadTo identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients healthAs required, to provide routine nursing care to patients in accordance with clinical based evidence and NICE guidelines.As required, to diagnose and manage acute and chronic conditions with support from the clinical team.To follow medication for patients in accordance with PGDs and own scope of practiceTo review patient medications to enhance complianceTo provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosageTo request pathology services and process pathology results as requiredTo oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as requiredTo maintain accurate clinical records in conjunction with current legislationTo ensure read codes are used effectively by all members of the nursing teamTo ensure the appropriate maintenance and use of chronic disease registersTo develop, implement, embed and participate in well woman and well man clinicsTo ensure compliance with the practice chaperone policyTo prioritise health issues and intervene appropriatelyTo support the team in dealing with clinical emergenciesTo recognise, assess and refer patients presenting with mental health needsTo implement vaccination programmes for adults and childrenAbility to provide travel advice and vaccinationsTo liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)The ability to recognise and respond appropriately to safeguarding concerns for children and vulnerable adultsTo support the clinical team with all safeguarding matters, in accordance with local and national policiesTo understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriatelyTo deliver opportunistic health promotion where appropriate

Develop and review audit protocols and process for the practice, effectively utilising the audit cycleDevelop, review and implement nursing protocols in conjunction with the partnersTo develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practiceSupport all members of the nursing team, providing guidance when necessary, and training needs analyses as requiredParticipate in local initiatives to enhance service delivery and patient careSupport and participate in shared learning within the practice in order to improve patient careContinually review clinical practices, responding to national policies and initiatives where appropriateParticipate in the review of significant and near-miss events applying a structured approach, i.e., root cause analysis (RCA)Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation
