We are seeking a highly motivated, enthusiastic and proactive Practice Nurse to join and enhance our friendly and supportive GP practice team. We are committed to delivering high quality, patient centred care to our patient population.
This is an excellent opportunity for an experienced Practice Nurse, with a strong background in diabetes care. The successful candidate will play a key role in supporting patients with long-term conditions, alongside delivering a broad range of treatment room nursing duties.

Main duties of the job
To work as part of the nursing team, providing safe, effective and reactive nursing services to the practice population.
As a qualified nurse you will be required to work autonomously within your areas of competence and within the requirements of the NMC Code of conduct.
To work as part of the practice multidisciplinary team to develop an effective, quality driven practice nurse service, ensuring reference to practice agreed protocols and guidelines to deliver nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.
To support the management team in the reviewing of clinical policy and procedure.
Working autonomously and using sound clinical judgement and knowledge to provide a wide range of nursing services within agreed practice protocols, guidelines (both local and national) and PGDs

We are a busy, friendly, and supportive training practice with a team of 6 GPs. The practice operates from a main site in Coltishall and a smaller branch surgery in Spixworth, serving a largely rural population of approximately 9,000 patients. Situated in the beautiful Norfolk Broads, we pride ourselves on delivering high-quality, patient-centred care within a welcoming community setting.
Job description Job responsibilities Duties and Responsibilities
Working autonomously within agreed practice protocols, local and national guidelines and PGDs, the Practice Nurse will deliver a high standard of patient-centred care across a wide range of clinical areas.
Clinical Responsibilities:
Management and monitoring of patients with diabetes
Supporting patients with long-term conditions including asthma, COPD, hypertension, CHD and CKD
Conducting chronic disease reviews and developing patient-centred care plans
Delivering treatment room services including wound care, immunisations, cervical cytology.
Undertaking health promotion and lifestyle advice to support behaviour change
Providing cardiovascular risk assessment and management, including lifestyle and medication compliance support
Delivering sexual health services, including contraception, family planning and advice
Supporting womens health (cervical screening, menopause advice, continence) and mens health awareness
Carrying out clinical investigations (e.g. venepuncture, ECGs, urinalysis, BP monitoring) and acting on results appropriately
Administering and supporting immunisation programmes (childhood, travel and seasonal)
Providing wound care assessment and ongoing management
Supporting anticoagulation monitoring and safe management of patients on warfarin
Delivering care under Enhanced Services, including health checks and minor illness/injury support
Responding appropriately to urgent presentations within the practice
Patient Care & Communication:
Promote patient self-management and shared decision-making
Identify safeguarding concerns and act in accordance with policy
Recognise and adapt to barriers in communication
Professional & Team Responsibilities:
Maintain accurate, contemporaneous clinical records in line with NMC standards
Work collaboratively with the wider multidisciplinary team
Participate in audits, quality improvement and service development
Maintain compliance with infection control, CQC standards, and practice policies
Ensure confidentiality and information governance standards are upheld
General Responsibilities:
Prioritise workload effectively and respond to urgent needs
Participate in mandatory training, professional development and revalidation
Contribute to team cover during periods of absence
Support continuous improvement and innovation within the practice

Experience working within primary care, general practice, or community nursing Proven experience in managing patients with diabetes Competence in treatment room skills (e.g. wound care, immunisations, cervical screening, venepuncture) Ability to work autonomously and make sound clinical decisions within scope of practice Strong organisational skills with the ability to prioritise workload effectively Understanding of safeguarding, infection control, and confidentiality requirements Commitment to continuing professional development and high standards of patient care
Desirable
Qualification or training in chronic disease management. Experience using clinical systems such as Emis Qualification or training in chronic disease management. Evidence of continued professional development and training.
Qualifications Essential
Registered Nurse with a valid NMC registration Excellent communication and organisational skills A compassionate, patient-centred approach to care Ability to work both independently and collaboratively within a multidisciplinary team
Desirable
Experience working within primary care or community healthcare settings Knowledge of local public health priorities and population health needs Awareness of current issues and developments within the wider healthcare system
Qualifications Essential

Recognised qualification or specialist training in diabetes care Experience working in a GP practice setting Experience in cardiovascular disease management and risk reduction Knowledge of Quality and Outcomes Framework (QOF) and Enhanced Services Experience in anticoagulation monitoring (e.g. INR/warfarin management) Experience participating in clinical audit and quality improvement initiatives
