Job summary
We are seeking an experienced Practice Nurse to join our friendly and wellestablished clinical team. You will play a key role in delivering highquality, patientcentred nursing care and supporting the management of longterm conditions and health promotion across the practice population.
Main duties of the job
Deliver evidencebased nursing care in line with NICE guidelines
Manage chronic disease clinics, including respiratory conditions (asthma/COPD)
Provide wound care, including compression therapy and Doppler assessments
Undertake cervical cytology, immunisations, and health checks
Diagnose and manage acute and chronic conditions within scope of practice
Prescribe under PGDs and review patient medications
Support Infection Prevention & Control processes, audits, and compliance
Maintain accurate clinical records and ensure effective clinical coding
Support safeguarding of adults and children
Assist with minor surgery when required
Liaise with external services to support vulnerable and housebound patients

Person Specification
Qualifications Essential
Registered Nurse with current NMC registration Previous experience working as a Practice Nurse or in Primary Care Competence in immunisations, cervical screening and chronic disease management Strong communication and organisational skills Ability to work independently and as part of a multidisciplinary team Confident IT skills and experience using clinical systems Understanding of confidentiality, safeguarding and professional governance
Desirable
Qualification or experience in asthma and-or COPD management Wound care and compression therapy experience Nonmedical prescribing qualification (or working towards) Experience supporting Infection Prevention & Control initiatives Experience working within a PCN
