About this role

Job summary

Llandaff North Medical Centre is looking for a primary care pharmacist to join their clinical team in a new role to the practice, on a fixed term basis in the first instance. We are looking to appoint a pharmacist to work for 16 hours over two days each week, for a 9 month term, with potential for extension dependent on renewal of external funding (funded through the primary care workforce fund).

Main duties of the job

Candidates must be qualified pharmacists registered with the GPhC and ideally would have prior experience in primary care.

The role will include but not be limited to the following areas and as a new role, can be developed to suit the skills of the appointee and meet the needs of the practice

Take an active lead in the prescribing activities of the practice and day to day medicine management, working closely with the prescribing clerk to ensure the smooth running and efficiency of all prescribing activity

Undertake planned medication reviews with patients, both on the telephone and where appropriate face to face, in line with the practice medication review policy

Undertake reviews in chronic disease, including hypertension and cardiovascular disease, respiratory disease and other areas as experience allows

Participate in the practice prescription query process

Take part in medicine reconciliation including through review of hospital discharge summary letters and clinic letters

Participate in medicine management audits including those suggested as part of the MMIS and other practice or health board defined projects

Engage in practice education and provide mentorship for the practice prescribing clerk as well as supporting the wider clinical team in aspects of prescribing

A weekly supervision hot review style review will be scheduled with a senior GP for ongoing clinical support and continued development of this new role

The practice area is part of the Cardiff West Cluster and primarily serves the community in Llandaff North, Gabalfa and Whitchurch, with some wider patients in the Fairwater, Danescourt and Llandaff area, providing a varied, rewarding and interesting patient caseload.

c6500 patients and stable list size

Currently 2 partners, 4 salaried GPs - 1 in a temporary post, 2 GP registrars, 2 nurses, 1 HCA and an excellent administration team, including a designated prescription clerk.

Daily morning coffee meeting of the clinical team, alongside structured rotational clinical meetings for training and multi-disciplinary discussion.

Training practice for GP registrars - usually 1-2 registrars ST1-3

Person Specification

Qualifications Essential

Registered Pharmacist with the GPhC Strong knowledge of medicines management in primary care

Desirable

Independent Prescriber status or willingness to undertake training if not yet qualified.

Experience Essential

Experience working in a patient facing clinical role Excellent communication and consultation skills Ability to work autonomously and as part of an MDT Commitment to patient centred care and quality improvement

Desirable

Previous experience in general practice or primary care Experience of developing or improving prescribing processes Experience using GP clinical systems i.e. EMIS Audit or service improvement experience
