Job summary
We are recruiting for an experienced Practice Nurse to join our amazing Nursing team which work across three surgeries.
You will be providing high quality care based on the needs of our patient population which includes treatment, screening, preventative care and patient education.
Main duties of the job
We welcome applications from applicants that have demonstrable experience in either asthma, COPD or diabetes reviews along with experience in chronic disease management
You will support and manage patients with chronic disease, running specific clinic sessions and maintaining robust call and recall systems.

Clinical Practice
Experience in either asthma, COPD or diabetes reviews is essential
To manage patients with chronic disease, running specific clinic sessions and maintaining robust call and recall systems
Experience in chronic disease management
Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule
Wound care
Provide travel health advice and administer appropriate vaccinations
Provide directly and assist GPs with contraceptive services
Break bad news to patients/parents/carers and provide appropriate ongoing support to meet identified needs. Signpost patients to relevant support networks
Act as a chaperone when required
Undertake health promotion to help educate patients
Select, carry out and interpret results of specific tests
Undertake anticoagulation clinics to include interpretation of blood tests and dosing
Communication
Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment
Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
Anticipate barriers to communication and take action to improve communication
Estimate and maintain effective communication with individuals and groups within the practice environment and with external stakeholders
Act as an advocate when representing patients
Administration
To maintain accurate patient records and enter onto the system using agreed Read Snomed Codes
Ensure accurate completion of all necessary documentation associated with patient care
To collect data and participate in regular Practice audit and research projects
To contribute to the preparation of any Practice development plans
Delivering a Quality Service
To maintain own annual administration with the NMC and adhere to Code of Professional Conduct
To maintain a safe and therapeutic environment for all PHCT staff, patients, carers and co-workers from other agencies. Ensure all statutory regulations e.g. Health and Safety, COSHH etc. are implemented and adhered to
To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice
To maintain own continuous professional development and ensure PDDP kept up to date in line with the requirements of the practice
Attend and participate in staff and Practice meetings and in house clinical training sessions
Any other reasonable duties that may be required from time to time
Recognise and work within own competence and professional code of conduct as regulated by the NMC
Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
Prioritise, organise and manage own workload in a manner that maintains and promotes quality
Evaluate the patients response to health care provision and the effectiveness of care
Support and participate in shared learning across the practice and wider organisation
Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedure and local guidance
Work within policies regarding family violence, vulnerable adults, substance abuse and addictive behaviour, and refer as appropriate
Team Working
Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working
Delegate clearly and appropriately, adopting the principles of safe practice and assessing competence
Ensure clear referral mechanisms are in place to meet patient need
Prioritise own workload
Participate in team activities that create opportunities to improve patient care

In the course of seeking treatment, patients entrust practice staff with, or allow us to gather, sensitive information in relation to their health and other matters.
Comply with Legislation with regards to data protection act and ensure confidentiality is maintained at all times
The post-holder must regard all information relating to patients and their carers, practice staff and other healthcare workers (as well as information relating to the practice as a business organisation) as strictly confidential
