We are looking for an enthusiastic practice nurse to join our friendly team and play a vital role in delivering high-quality, patient-focused healthcare to our diverse practice population.

About the Role

As a Practice Nurse you will be at the heart of our clinical team, providing comprehensive care for patients with a wide range of needs. A key focus of the role is delivering evidence-based care for individuals with long-term conditions as well as promoting and delivering preventative healthcare initiatives that improve overall community wellbeing.

You will use your clinical expertise and sound judgement to ensure safe, effective decision-making while delivering compassionate, personalised care. This is an opportunity to truly utilise and develop your skills in a collaborative and supportive environment.

Main duties of the job

About us

Milborne Port Surgery is a purpose built, modern GP practice in pretty Somerset village, close to the Dorset border. Our smaller branch surgery is in the village of Templecombe.

We are a very friendly team and we arrange regular staff social events and whole-practice meetings. We have a wide range of staff working at the practice including GPs, Pharmacists, a Paramedic, Practice Nurse and HCAs. Our patient population is just under 7500.

We are looking for a candidate who fits in well with the ethos of the practice.

Job description Job responsibilities

The post holder is responsible for the delivery of general practice nursing care to the whole practice population. MAIN DUTIES AND RESPONSIBILITIES

1. Clinical

To provide nursing services to patients of the practice including treatment, screening and advice.

Duties to include:

Dressings and wound management

Cervical smears and swabs

Child and adult immunisation

Travel advice and vaccinations

Family planning (administration of contraceptive injections and routine contraceptive pill reviews)

Administration of other injections, e.g. B12, hormone injections

Removal of surgical sutures/clips

Delivering chronic disease management to patients (including in clinic settings) in accordance with good practice and to deliver key performance indicators. The post holder with work with other health professionals to diagnose, monitor, manage and treat these conditions using appropriate and agreed management plans. These services may be delivered in organised clinics and will include COPD, asthma, and diabetes.

Providing clinical care in line with agreed Practice, local and national protocols or guidance.

Data gathering and delivery of clinical care to help the practice achieve its QOF goals.

Recording clear and contemporaneous consultation notes to agreed standards using the agreed computerised systems for all patient information.

Giving health promotion advice and education for all patients and signposting to other available services.

The post holder will be expected to undertake all or some of the above duties in the branch site settings if required.

2. Personal Development and Professional responsibilities

To provide a high standard of professional conduct and nursing care at all times in accordance to the NMC Professional Code of Conduct.

Ensure all records are accurate, complete and permanent in line with practice policy and NMC guidance.

Participate in safeguarding processes relating to children and adults registered with the practice.

Be accountable for all decisions made and care provided to patients.

Be aware of personal limitations in knowledge and practice and seek appropriate advice from the doctors.

Monitor effectiveness of own clinical practice through audit and peer review.

Participate in continuing professional development to ensure that up-to-date evidence-based knowledge and competencies are maintained.

Work collaboratively with colleagues within and outside of the practice.

To take responsibility for own developmental learning and performance. To identify, with the support of the Practice Manager, appropriate areas of personal development.

Participate in the practice appraisal scheme.

To attend and participate in practice meetings including clinical, educational and developmental practice meetings. To undertake training as required including mandatory and statutory training.

To recognise and understand the roles and responsibilities of the whole primary health care team.

To strive to maintain quality within the practice.

To alert line manager and other team members to issues of quality and risk.

To assess own performance and take accountability for own actions.

To work effectively with individuals in external agendas to meet patient needs.

3. Health and Safety / Risk Management

The post holder will be trained in and expected to use the personal security systems available within the practice.

The post holder must comply at all times with practice Health and Safety policies, in particular by following agreed safe working procedures and reporting clinical incidents using Milborne Port Surgery's Significant Event Reporting system.

To use computers safely and appropriately.

The practice is registered under the Data Protection Act 1984. The post holder will comply with the Data Protection Act and the Access to Health Records Act and the practice policies regarding information governance at all times. You must not at any time use the personal data held by the practice for a purpose not described in the Register entry or disclose such data a to a third party. If you are in any doubt regarding what you should and should not do in connection with the Data Protection Act, then you must contact your line manager.

Person Specification

Experience Essential

Experience of working as a nurse within the NHS. Experience of dealing with vulnerable patients.

Desirable

Experience of working within primary care. Experience of the Quality and Outcome Framework (QOF) in primary care. Experience in chronic disease reviews (e.g. performing reviews for patients with asthma, COPD and diabetes). Experience with managing respiratory conditions is particularly desirable for our current skill mix. Experience in the NHS vaccination schedule and skills in vaccination administration (childhood and adult vaccinations). Experience in giving travel health advice. Cervical cytology training and experience. Experiencing in contraceptive services, e.g. administration of contraceptive hormone injections and routine contraceptive pill reviews. Confidence in phlebotomy.

Qualifications Essential

NMC-approved nursing degree or nursing degree apprenticeship qualification. Registration with NMC.

Desirable

Qualification in phlebotomy. Additional training in vaccination administration, cervical cytology and wound care. Qualifications in chronic disease management, e.g. diploma in respiratory condition management or diabetes management.
