Job summary

We are seeking an enthusiastic and motivated Practice Nurse to join our friendly, patient-focused primary care team. This is an excellent opportunity to work within a supportive and forward-thinking GP practice committed to delivering high-quality care to our local population.

We are ideally looking for an experienced Practice Nurse; however, we would also warmly welcome applications from nurses with a different clinical background who can demonstrate transferable skills and a willingness to develop within primary care.

The successful candidate will play a vital role in delivering nursing services, health promotion, and chronic disease management within a busy but rewarding environment.

Hours: 25.5 per week (Wednesday, Thursday & Friday)

Job Purpose

To provide high-quality, patient-centred nursing care within the practice, supporting the delivery of primary care services, chronic disease management, health promotion, and treatment room services.

The post holder will work collaboratively with the multidisciplinary team to ensure safe, effective, and efficient care for the practice population.

Key Responsibilities

Clinical Duties

Deliver high-quality, evidence-based nursing care to patients within the practice settingLead and contribute to chronic disease management clinics (e.g. diabetes, COPD, asthma), in line with local and national guidelinesUndertake cervical cytology (smear tests) in accordance with national screening programmesAdminister immunisations and vaccinations in line with current protocols and Patient Group Directions (PGDs)Provide wound care, dressings, and a full range of treatment room servicesAssess, plan, implement and evaluate patient careHealth Promotion & Patient EducationPromote health and wellbeing through patient education and lifestyle adviceSupport national and local health promotion initiativesEncourage patients to engage in self-care and preventative health programmesInfection Prevention & ControlAct in accordance with infection prevention and control policiesMaintain a safe clinical environment for patients and staffParticipate in audits and compliance activities where requiredAdministrative & Record KeepingMaintain accurate, contemporaneous, and legally compliant clinical recordsUse clinical systems (e.g. EMIS) effectively to support patient careContribute to data collection and quality improvement initiatives (e.g. QOF)Team Working & CommunicationWork collaboratively with GPs, nurses, healthcare assistants and the wider multidisciplinary teamCommunicate effectively with patients, carers, and colleaguesParticipate in practice meetings and contribute to service developmentProfessional ResponsibilitiesMaintain active NMC registration and adhere to the NMC CodeEngage in continuing professional development (CPD)Participate in appraisal and revalidation processesWork within own scope of competence and seek support when requiredTraining & DevelopmentParticipate in a structured induction programmeEngage in ongoing training including chronic disease management, cytology, and immunisations (if required)Access mentorship and clinical supervisionTake part in internal and external CPD opportunitiesGeneral ResponsibilitiesComply with practice policies and proceduresAdhere to confidentiality, data protection, and information governance requirementsSupport equality, diversity, and inclusion within the practiceContribute to a positive and supportive working environment

Person Specification

Experience Essential

Registered Nurse (NMC) Strong communication and interpersonal skills Patient-centred approach Ability to work both independently and as part of a team

Desirable

Experience in primary care or community nursing Chronic disease management experience Cytology and immunisation qualifications Familiarity with EMIS
