Job summary
We have an exciting opportunity for a Practice Nurse to join our Nursing team at a dynamic expanding GP Practice in the heart of Surrey.

Practice Nurse experience preferred but willing to offer training for the right candidate.
Main duties of the job
The Practice Nurse is responsible for providing nursing services to the practice population, within the boundaries of the role. Successful Practice Nurse will support patients to be healthy, monitor patients with long-term conditions, and deliver health prevention, promotion and screening activities. This will include but is not limited to
Management of chronic disease Cervical Cytology Childhood Immunisations Administer a range of adult vaccinations. Wound care ECG's Hypertension management Actively participate in achieving QOF and Enhanced Services targets and recall demands Participate in practice audit as directed by the Clinical Lead/Practice Manager Support and participate in shared learning within the practice To play an active part of the clinical team at the practice attending regular nursing, education and other meetings.

The Practice Nurse will interact directly with patients, providing high quality care in accordance with specific Practice guidelines and protocols.

Chronic Disease Management : Special Interest/ responsibilities e.g in Asthma / COPD / Diabetes or Coronary Heart Disease and Contraception.
Cytology
Family Planning
Wound care
Travel Clinic and vaccinations
ECGs
Asthma/Peak Flow Readings/Spirometry
Hypertension Management
Routine immunisations/ Childhood immunisations
Requesting pathology tests, for example urine culture, swabs
Following agreed clinical protocols with referral to Lead Nurse or GPs as appropriate
Deliver opportunistic health promotion where appropriate.
Participate in practice audit as directed by the Clinical Lead/Practice Manager.
Participate in local initiatives to enhance service delivery and patient care.
Support and participate in shared learning within the practice.

Person Specification
Qualifications Essential
NMC Registered Adult Nurse with 3+ years experience
