About this role

Job summary

An exciting opportunity has arisen for a Band 6 Community Mental Health Nurse at the North Sheffield Community Mental Health Team. You will provide a range of mental health nursing intervention as part of a supportive and dynamic multi disciplinary team.

Main duties of the job

You will need to possess excellent communication skills and be able to work independently and as part of the wider team in order to assess, plan and evaluate care. You should have a positive approach to meeting the needs of our service users and be motivated, empowered and enthusiastic to both learn and share.

You will also likely to be expected to work as part of the duty system which operates within the general working hours of 09:00 to 17:00 Monday to Friday on a rotational basis. This 'mini team' is supported by other clinical professional and management.

Job description Job responsibilities

The post holder will work within a multi-disciplinary service and core responsibilities will include:

Acting as a Named Worker for service users Comprehensive assessment of new referrals Carrying a varied caseload defined by need Contributing to a duty system on a rota basis Contribute to Multidisciplinary service development

Clinical

Carry out comprehensive assessments of an individuals health and social care needs in a variety of settings and agree the most appropriate course of action. This includes first presentation to service. To assess, evaluate and manage clinical risk To formulate a plan of care based on identified need Deliver evidence based interventions as appropriate Review effectiveness of care plan at regular intervals To work within safeguarding processes, including where required to act as safeguarding investigator / manager. To work collaboratively with service users, carers and partner agencies To maintain computer held care records in an accurate and timely fashion Undertake all work in a culturally sensitive manner Act in accordance with Care Management System, Acute and Schedule Care Pathways as appropriate. Develop effective working relationships with Primary Care & Voluntary Sector colleagues

oWorking knowledge and understanding of MHA/MCA oKnowledge and understanding of appropriate therapeutic approaches and engagement strategies oKnowledge and understanding of PDR and supervision processes as a supervisee oDemonstrable risk assessment and risk management skills with evidence of positive risk taking

Additional skills /training in psychosocial interventions for people with SMI, CBTp and family interventions oRelevant Health or Social Care professional qualification - Nursing
