About this role
Job summary
Deliver hospital-level care where it can make a real difference: in a patient's own home.
RUH Hospital at Home (RUH@Home) is our consultant-led virtual ward, supporting patients who would otherwise need to be admitted to hospital, or who can safely return home earlier with the right clinical support around them. The service works closely with primary, community and social care partners to ensure safe transitions of care.
You will join a multidisciplinary team of doctors, nurses, therapists, therapy assistants and healthcare assistants providing personalised acute care in patients' homes, seven days a week between 08:00 and 18:00.
As a nurse within RUH@Home, you will have the opportunity to work with greater autonomy, assessing patients with acute and complex needs, developing individualised care plans, delivering evidence-based interventions and identifying and responding to changes in patients' conditions.
You will work independently in the community, but you will never be working in isolation. You will be part of a wider multidisciplinary team and work closely with primary, community and social care colleagues to ensure patients receive safe, coordinated care.
If you enjoy variety, autonomous working and building meaningful relationships with patients, while using your clinical skills to help people avoid an unnecessary hospital stay or return home sooner, this could be a fantastic next step in your nursing career.
Main duties of the job
In this key role, you will:
Assess, plan, deliver and evaluate individualised nursing care for patients with acute and complex needs in their own homes. Work autonomously, using your clinical judgement to recognise deterioration, respond appropriately and escalate concerns when required. Deliver safe, effective and compassionate care, including the administration of medicines and treatments. Work closely with the RUH@Home multidisciplinary team and wider health and care partners to provide coordinated, patient-centred care. Support admission avoidance and early supported discharge, helping patients receive high-quality care safely at home. Involve patients, families and carers in care planning, providing advice and education to promote independence and informed decision-making. Maintain clear and accurate clinical records and contribute to patient safety, quality improvement and service development. Support the supervision and development of students, healthcare support workers and new colleagues as appropriate
