About the Role? As a Mob Link Social Health Care Coordinator, you will provide culturally safe, person-centred, short-term coordination for Aboriginal and Torres Strait Islander people facing barriers to health and social services. Working alongside clients, you will identify their priorities, connect them with suitable supports and facilitate warm referrals to services that can meet their longer-term needs, including secure housing, mental health, Domestic and Family Violence, and other social health supports.

The position aim is to reduce the significant points of risk in a person’s journey through the health care system at hospital entry and discharge, and in transit from primary care in and out of specialist, allied health, housing, mental health, domestic and family violence, and other social health services.

Your day-to-day duties will include:

• Build culturally safe engagement with clients and identify their priorities, strengths, service access barriers and immediate coordination needs.

• Provide short-term, goal-focused coordination for clients requiring support to navigate health and social service systems.

• Identify appropriate internal and external services and clearly explain available options so clients can make informed choices.

• Action referrals, share relevant information with consent, and follow up to confirm that referrals have been received and connection has occurred.

• Coordinate with hospitals, primary care, IUIH services and community providers to support safe transitions and reduce duplication or gaps in care.

• Advocate with and for clients to address practical access barriers and promote culturally safe, responsive service engagement.

• Maintain clear, timely documentation of client goals, referrals, follow-up actions, outcomes and transition plans.

• Complete a planned transition from Mob Link once the client is connected to appropriate ongoing services, or when agreed short-term coordination goals have been met.

About You?

• Current C Class Drivers Licence (QLD)

• Current Criminal History Check.

• Current Working with Children’s Check (Qld Blue Card).

• Minimum Certificate IV in Social Work / Social Services.
