About this role
Provide daily oversight of clinical case management team to insure timely evaluation, development and implementation of individualized plan of care that insures at risk and chronically ill members receive necessary services and interventions that impact self-management, improved clinical outcomes and enhanced quality of life.
BSN degree required. Masters preferredMinimum one to three year previous management experience required Previous managed care experience in the areas of utilization management and/or case management preferred.Minimum one year nursing practice in a clinical settings, i.e hospital, nursing facility or home health. Working knowledge of Windows, Word, Excel and clinical case management software.Licensed to practice as a Registered Nurse in the state of New York with current registration. Certified Case manager preferred.
