About this role
Supervisor of social worker, students and interns. Provides appropriate and timely discharge from the nursing facility. Arrangements for follow-up care and referrals to community resource as needed. This responsibility is to be carried out with an understanding of Medical Social Work concepts, practices and procedures.
Education:
Masters Degree in Social WorkExperience:
Extensive fast paced discharge- planning experience.Supervisory experience preferred Certification and Licensure:
LCSW Preferred Skills:
Demonstrate excellent written and spoken skillsBilingual Spanish/English, preferred Able to work varying schedules, including weekends and eveningsComputer knowledge sufficient to access data, run reports, update, clinical programs. Working knowledge of Microsoft Windows, Excel and PowerPoint. Experience and judgment to plan and implement
