About this role

Job summary

This is an exciting opportunity to join Runcorn Primary Care Network (PCN) and be part of the start of our new Neighbourhood Health team, employed by our local federation, GPHC. The initial focus will be on developing a new Neighbourhood Multidisciplinary Team (MDT) approach, bringing together professionals from multiple health, care and community organisations around the needs of local people. As Care Coordinator, you will play an important role in connecting services, supporting individuals and helping colleagues from different clinical and non-clinical backgrounds work together effectively as we develop this new way of working.

This is a permanent role with significant long-term potential to grow and evolve alongside the neighbourhood model, giving you the opportunity to help shape both the MDT and the wider development of neighbourhood services and hubs. Main duties of the job

Key Priorities

Ensure the efficient organisation and coordination of MDT meetings, accurate tracking of patient pathways, timely collection and recording of information, and effective communication between members of the MDT and relevant partner organisations

Maintain accurate and up-to-date records of patients being considered or managed through relevant pathways

Work closely with clinicians and other members of the multidisciplinary team

Contribute to the delivery of a high-quality, organised and responsive service, supporting the MDT to meet relevant targets and improve patient care

Escalate delays, concerns or issues appropriately to the Neighbourhood Clinical Lead or relevant clinical professional.

Act as a key administrative point of contact for MDT-related queries

Job description Job responsibilities

MDT Coordination

Coordinate regular MDT meetings, including arranging dates, preparing agendas and circulating relevant patient lists and information.

Attend MDT meetings and accurately record discussions, decisions, outcomes and actions.

Maintain an effective system for tracking actions arising from MDT meetings and ensure these are followed up within agreed timescales.

Ensure that relevant clinical and administrative information is available to support effective MDT discussions.

Act as a key administrative point of contact for MDT-related queries.

Patient Pathway Tracking

Maintain accurate and up-to-date records of patients being considered or managed through relevant pathways.

Monitor patient pathways and identify potential delays or outstanding actions.

Escalate delays, concerns or issues appropriately to the Neighbourhood Clinical Lead or relevant clinical professional.

Ensure that information relating to patient pathways is accurately recorded on appropriate electronic systems.

Support the MDT in monitoring progress against relevant targets and timescales.

Administration and Information Management

Provide comprehensive administrative and secretarial support to members of the MDT and wider clinical teams.

Maintain accurate electronic records and databases in accordance with organisational requirements.

Collate information and produce reports to support audits, performance monitoring and service development.

Ensure that documentation and information are appropriately maintained and readily available to authorised staff.

Support the development and maintenance of effective administrative processes and tracking systems.

Communication and Liaison

Communicate effectively with clinicians, nurses, allied health professionals and other members of the multidisciplinary team.

Liaise with external organisations, support services and other relevant agencies where required.

Respond appropriately to queries relating to MDT processes and patient pathways.

Establish and maintain effective working relationships with colleagues and partner organisations.

Communicate sensitive and confidential information in accordance with relevant policies and procedures.

Collaborative Working Relationships

Recognise the roles of colleagues within the organisation and their contribution to patient care.

Demonstrate the ability to work effectively as part of a multidisciplinary team.

Foster and maintain effective working relationships with clinicians, nurses, allied health professionals, administrative colleagues and external organisations.

Liaise appropriately with members of the wider health and social care system where required.

Recognise personal limitations and seek advice or escalate matters to the appropriate colleague when necessary.

Contribute positively to the development and effectiveness of the neighbourhood MDT.

Knowledge, Skills and Experience Required

Previous administrative experience, preferably within a healthcare or similar environment.

Experience of working with data, electronic records and information systems.

Experience of working within or supporting a multidisciplinary team would be desirable.

Good organisational and time-management skills, with the ability to manage competing priorities.

Good written and verbal communication skills.

High level of attention to detail and accuracy.

Understanding of confidentiality, information governance and data protection requirements.

Education, Training and Development

Participates in relevant training and development opportunities identified through appraisal and service requirements.

Demonstrates an understanding of relevant practice, PCN and organisational policies and procedures.

Person Specification

Skills and Knowledge Essential

Excellent written and verbal communication skills. Strong organizational skills with the ability to manage multiple caseloads and meet deadlines. Proficient in Microsoft Office (Word, Excel, Outlook). Ability to plan and co-ordinate high-level meetings (e.g., MDTs). Highly motivated to work collaboratively as part of a wider multidisciplinary team.

Desirable

Experience handling difficult or sensitive conversations with patients and families. Advanced data analysis and data extraction skills using Excel.

Experience Essential

Experience working within a health, social care, or voluntary sector environment. Experience handling patient/client-facing interactions and co-ordinating patient care pathways. Experience using clinical IT databases and record systems. understanding of patient confidentiality (GDPR) and safeguarding policies.

Desirable

Experience within an NHS Primary Care Network (PCN) or GP Practice. Pre-existing knowledge of local community, housing, and social care assets.

Qualifications Essential

GCSE or equivalent in English and Maths (Grades A*C / 49). NVQ Level 3 or 4 (or equivalent experience) in health, social care, or business administration or equivalent experience. Evidence of continuous professional development.

Desirable

Formal Care Coordination or Health Coaching certification.
