About this role

Job summary

An exciting opportunity to join Runcorn Primary Care Network (PCN) and be part of the start of our new Neighbourhood Health model, employed by our local federation, GPHC. The initial focus will be on developing a new Neighbourhood Multidisciplinary Team (MDT) approach, bringing together colleagues from across health, care and community services to work differently around the needs of our local population. As Clinical Lead, you will help shape this approach, working with multiple partner organisations and a wide range of clinical and non-clinical professionals to develop joined-up neighbourhood services and support the future development of our Neighbourhood Health Hubs.

This is a permanent position that reflects the strategic development and long-term vision of the programme. As the neighbourhood approach grows, so too will the team and the opportunities within it, with this role expected to develop alongside our evolving services. Main duties of the job

Key Priorities

Support the development of a new Neighbourhood Multidisciplinary Team approach, bringing together colleagues from across health, care and community services to work differently around the needs of our local population.

Provide expert clinical and professional leadership to the Neighbourhood multidisciplinary team, ensuring the delivery of high-quality, safe, effective and person-centred care.

Take ownership of identified priorities of care, proactively driving actions and addressing challenges and delays through to resolution

Take responsibility for clinical governance, quality improvement, service development and clinical oversight within the Neighbourhood service

Provide leadership, direction and support to members of the MDT, promoting professional development, effective supervision, workforce planning and the delivery of a high-performing multidisciplinary service

Where appropriate, the post holder will maintain a clinical caseload alongside their leadership responsibilities, working within their professional scope of practice, competence and relevant professional standards

Communicate expectations clearly, professionally and respectfully across professional and organisational boundaries, building and maintaining effective working relationships with primary care, community, secondary care, social care and other partner organisations.

Primary Responsibilities

Clinical Leadership

Provide visible clinical leadership and direction to the Neighbourhood MDT.

Provide expert clinical advice and support to MDT members and other healthcare professionals.

Promote high standards of clinical practice and ensure care is delivered safely, effectively and in accordance with relevant professional standards.

Maintain a clinical caseload where appropriate to the requirements of the role and professional discipline.

Support complex clinical decision-making and promote evidence-based practice.

Promote a culture of learning, professional accountability and continuous improvement.

Support colleagues to recognise and manage clinical risk and escalate concerns appropriately.

Clinical Governance and Quality

Lead and promote effective clinical governance arrangements across the MDT.

Ensure that clinical practice is consistent with relevant legislation, professional standards, organisational policies and national guidance.

Lead and contribute to clinical audit, quality improvement and service evaluation.

Identify, assess and manage clinical and operational risks within the service

Support the investigation and review of incidents, complaints and other patient safety concerns.

Ensure learning from incidents, complaints, audits and reviews is appropriately shared and implemented.

Promote and monitor compliance with safeguarding requirements and patient safety standards.

Contribute to the development, implementation and review of clinical policies, procedures and guidelines.

Monitor quality indicators and support the MDT to achieve agreed standards and outcomes.

Service Development and Improvement

Lead the development and implementation of service improvement initiatives within the Neighbourhood MDT.

Identify opportunities for innovation and improvements in the delivery of patient care.

Develop and implement clinical pathways that support integrated and person-centred care.

Work with relevant stakeholders to review existing pathways and identify opportunities to reduce duplication, improve access and enhance patient outcomes.

Contribute to strategic planning and the development of the Neighbourhood service.

Use clinical evidence, data, patient feedback and service intelligence to inform service development.

Lead or contribute to change programmes and the implementation of new models of care.

Evaluate the effectiveness of service developments and make recommendations for further improvement.

MDT Coordination and Clinical Decision-Making

Chair the Neighbourhood MDT meetings

Ensure MDT meetings are structured, focused and support effective multidisciplinary decision-making.

Promote collaborative working and ensure that appropriate professional perspectives are considered in clinical decision-making.

Support the MDT to develop coordinated care plans for patients with complex needs.

Ensure appropriate actions and decisions arising from MDT meetings are communicated and followed up.

Promote effective communication across all Neighbourhood partners including wider health, social care and voluntary sector services.

Ensure appropriate escalation of complex clinical issues and risks.

Workforce Leadership and Development

Participate in appraisal, supervision and performance development processes as appropriate.

Identify training and development needs within the Neighbourhood team and support appropriate learning opportunities.

Promote a positive learning environment and encourage staff to develop their skills and professional practice.

Support succession planning and development of clinical leadership capacity within the team.

Address performance or professional concerns appropriately and in accordance with relevant organisational procedures.

Education, Training and Development

Demonstrate commitment to continuous professional development and maintain appropriate professional registration.

Participate in relevant training and development opportunities.

Provide or facilitate clinical education and training for MDT members and wider healthcare professionals.

Contribute to relevant PCN, neighbourhood and multidisciplinary educational networks.

Maintain an up-to-date knowledge of relevant national policy, clinical guidance and developments in integrated care.

Collaborative Working Relationships

Recognise and value the contribution of all professional groups involved in patient care.

Foster effective working relationships across primary care, community services, secondary care, social care, mental health, voluntary sector and other partner organisations.

Work collaboratively across organisational boundaries to improve patient pathways and outcomes.

Person Specification

Qualifications Essential

Registered general adult nurse with current NMC registration. Advanced practice qualification e.g. Specialist practitioner qualification. Evidence of post-registration teaching, assessing, and ongoing professional development.

Desirable

Independent/Supplementary Prescriber (V300) qualification. Formal leadership or management qualification.

Skills and Knowledge Essential

Advanced clinical assessment and diagnostic reasoning skills. Comprehensive knowledge of safeguarding procedures, clinical governance, and CQC compliance standards. High-level communication, negotiation, and change-management skills. Full UK driving licence and access to a vehicle for work purposes

Desirable

Knowledge of how to sue local demographic data to address health inequalities. An understanding of the local Health care structures, including how Integrated Neighbourhood Teams (INTs) interact with broader Place-Based Partnerships.

Experience Essential

Substantial post-registration experience working within a community healthcare or primary care setting. Proven experience in autonomous clinical decision-making and managing complex patient caseloads. Demonstrable experience providing clinical leadership or supervising multidisciplinary/nursing teams.

Desirable

Experience of clinical leadership within a PCN or Place setting. Experience of project management or leading service change. Experience supporting staff development or appraisal processes.
