Job summary
We are looking to recruit a motivated, experienced and compassionate Advanced Nurse Practitioner (ANP) to join our friendly and supportive team.
This is an exciting opportunity for an experienced ANP who enjoys working autonomously while being part of a collaborative and forward-thinking primary care team. The successful candidate will provide high-quality, patient-centred care and contribute to the continued development of our practice.
The role offers the opportunity to work alongside an experienced team of GP Partners, salaried GPs, practice nurses, HCAs and the wider multidisciplinary team in either a full time or part time role.
We are keen to support professional development and would welcome applications from ANPs with particular clinical interests that could complement our existing services.
Main duties of the job
Undertaking comprehensive clinical assessments and examinations
Diagnosing and managing a broad range of acute and chronic conditions
Developing and implementing appropriate treatment and management plans
Prescribing safely and appropriately within professional scope of practice
Providing health promotion, prevention and self-management advice
Recognising and managing patients whose condition requires escalation or referral
Working collaboratively with GPs, practice nurses, HCAs and the wider multidisciplinary team
Participating in clinical meetings, significant event reviews and practice development activities
Contributing to the supervision, education and development of students and other healthcare professionals where appropriate
Maintaining accurate, contemporaneous clinical records
Working within professional, legal and organisational standards and guidelines
Contributing to the ongoing development and improvement of primary care services
There will also be opportunities to develop specialist areas of interest, depending on experience and the needs of the practice.

St Thomas Surgery has around 13,000 patients and is a well-established, forward-thinking and successful training practice with a dynamic and supportive partnership.
We operate from one main surgery and two branch surgeries, one of which is a dispensing surgery, serving a diverse and engaged patient population. There are currently 4 GP Partners and 3 salaried GPs, working alongside our experienced practice nursing and healthcare assistant teams.
St Thomas Surgery is situated in the market town of Haverfordwest, in the stunning coastal county of Pembrokeshire. The successful applicant will be expected to:
Practise as an autonomous Advanced Nurse Practitioner within their professional competence and scope of practice
Provide safe, effective and evidence-based clinical care
Undertake advanced clinical assessment, examination and diagnosis
Develop, implement and evaluate individualised treatment and management plans
Prescribe and review medication appropriately, where qualified to do so
Recognise and respond appropriately to patients whose condition requires urgent or specialist assessment
Make appropriate referrals and liaise with secondary care and other healthcare professionals
Promote health, wellbeing and preventative healthcare
Maintain professional standards in accordance with the NMC Code and relevant legislation and guidance
Maintain accurate and appropriate clinical documentation
Participate in clinical governance, audit, quality improvement and significant event processes
Contribute to the development of practice policies, protocols and services
Work effectively as part of the multidisciplinary team
Participate in practice meetings and educational activities
Support the learning and development of students, trainees and colleagues where appropriate
Maintain continuing professional development and undertake relevant training to maintain and enhance clinical competence
Respect patient confidentiality and comply with all practice policies relating to information governance, safeguarding, infection control and health and safety
