We are seeking a compassionate, dedicated, and detail-oriented Certified Home Health Aide (CHHA) to join our hospice interdisciplinary team. This field-based role focuses on providing essential personal care and emotional support to terminally ill patients, ensuring comfort, dignity, and quality of life across the San Gabriel Valley (SGV) region. The ideal candidate will work independently in the field, confidently manage digital documentation, and possess a deep respect for diverse cultural backgrounds. Our Values: Put Patients First Empower Entrepreneurial Provider and Care Teams Operate with Integrity & Excellence Be Innovative Work As One Team Assist patients with activities of daily living (ADLs) including bathing, dressing, grooming, feeding, and oral hygiene under the direction of a Registered Nurse/DPCS. Provide skin care to prevent breakdown, assist with safe ambulation, transfers, and prescribed exercises, ensuring a safe and supportive environment. Monitor and record vital signs as directed. Document and immediately report any changes in the patient’s physical, mental, or emotional condition to the Case Manager or Supervisor. Use the HospiceMD EMR platform to log patient visits, care plans, and clinical narratives accurately and timely, in accordance with agency policy and regulatory standards. Travel reliably between assigned patient locations across the SGV territory. Track business travel and accurately submit monthly mileage invoices using Concur for reimbursement. Work collaboratively with nurses, social workers, chaplains, and families to carry out the comprehensive plan of care. Provide emotional support and companionship to patients and their loved ones.
