About this role

Job summary

The Senior Clinical Fellow will join the Cardiology Department and contribute to the delivery of safe, effective and patient-centred care at senior registrar level under consultant supervision.

The postholder will support the day-to-day management of patients on the Cardiology Ward and Coronary Care Unit. Duties will include daily ward rounds, patient assessment, coordinating investigations and treatment plans, supporting timely discharge and escalating clinical concerns appropriately.

The role will include participation in general and specialist outpatient clinics, including heart failure, arrhythmia and stable chest pain services. The postholder will also rotate through the cardiac catheter laboratory, with supervised opportunities to develop experience in cardiac catheterisation, pacing and cardiac devices, electrophysiology and ablation.

The postholder will support cardiac imaging services, including echocardiography and stress echocardiography, according to experience and competence.

They will participate in the agreed clinical and on-call rota, provide reasonable cross-cover and ensure safe clinical handover. The postholder will also contribute to teaching, audit, research, quality improvement and clinical governance.

The successful candidate will work closely with consultants, resident doctors, nurses, pharmacists, cardiac physiologists and the wider multidisciplinary team while complying with Trust policies and professional standards.

Main duties of the job

Main Duties and Responsibilities

Deliver safe, effective and patient-centred cardiology care at senior registrar level under consultant supervision. Assess, investigate and support the treatment of cardiology and general medical patients. Assist with the daily management of the Cardiology Ward and Coronary Care Unit, including ward rounds, treatment planning and discharge coordination. Review cardiology outliers and urgent referrals from the Emergency Department, Acute Medical Unit and other specialties. Participate in general cardiology, stable chest pain and relevant specialist outpatient clinics. Rotate through the cardiac catheter laboratory and support interventional cardiology within the postholder's competence. Support cardiac imaging, including echocardiography and stress echocardiography. Participate in the agreed on-call and weekend rota and provide safe clinical handovers. Work collaboratively with the wider multidisciplinary team. Maintain accurate records and contribute to clinical governance, audit, research, quality improvement and patient safety. Support teaching, supervision, mandatory training and continuing professional development. Undertake other appropriate duties agreed with the Clinical Lead and included in the approved work schedule.

Detailed Job Description and Main Responsibilities

The Senior Clinical Fellow will work at registrar level as part of the Cardiology medical team under the supervision of the consultant cardiologists. The postholder will contribute to general and specialist cardiology services across Ashford and St Peters Hospitals and may be required to work at other Trust locations in accordance with the approved work schedule.

The post provides broad experience in acute and elective cardiology while supporting safe, effective and timely patient care. The postholder will exercise an appropriate level of clinical autonomy, recognise the limits of their competence and seek senior advice promptly when required. All duties must be undertaken in accordance with General Medical Council standards, Trust policies, clinical guidelines and local procedures.

Inpatient cardiology and Coronary Care Unit

Contribute to the day-to-day management of the Cardiology Ward and Coronary Care Unit, working as the ward registrar under consultant leadership. Work collaboratively with consultants, resident doctors, nursing staff, pharmacists and allied health professionals to coordinate patient care. Participate in daily consultant-led ward rounds and undertake additional reviews according to clinical need. Assess new admissions and review patients with acute and chronic cardiovascular conditions. Establish differential diagnoses and arrange, review and interpret appropriate investigations. Develop and document clear treatment and management plans in discussion with the supervising consultant. Review cardiology outliers and provide registrar-level cardiology advice to clinical teams elsewhere in the Trust. Participate in board rounds, multidisciplinary discussions, clinical handovers and discharge-planning meetings. Support effective patient flow by identifying barriers to discharge and ensuring that investigations, medication and follow-up arrangements are completed promptly. Communicate clinical plans clearly to patients, relatives, carers, primary care teams and community services. Acute referrals, handover and rota participation

Assess urgent cardiology referrals from the Emergency Department, Acute Medical Unit, Same Day Emergency Care and inpatient specialties. Provide timely and evidence-based cardiology advice, supported by consultant input when required. Document clinical assessments, advice and management plans clearly in the electronic patient record. Escalate promptly when a patient requires consultant review, transfer to a cardiology bed, critical care support or urgent intervention. Participate in the agreed clinical, on-call and out-of-hours rota described in the approved work schedule. Outpatient cardiology

Participate in general and specialist cardiology outpatient clinics according to service requirements and the postholders experience. Contribute to clinics that may include general cardiology, heart failure, arrhythmia and stable chest pain services. Assess new and follow-up patients, review previous investigations and formulate appropriate management plans under consultant supervision. Cardiac catheter laboratory and procedural experience

Rotate through the cardiac catheter laboratory and support the safe delivery of diagnostic and interventional cardiology services. Participate in the assessment and preparation of patients before cardiac procedures. Cardiac imaging and diagnostic services

Support cardiac imaging and diagnostic services, including echocardiography and stress echocardiography. Review imaging and diagnostic findings in the context of the patients clinical presentation and discuss significant findings with the supervising consultant. Develop further knowledge of appropriate cardiac imaging investigations and their role in clinical decision-making. Clinical administration and communication

Maintain complete, accurate, contemporaneous and confidential electronic patient records. Complete discharge summaries, clinic letters, referral responses, procedure notes and investigation requests within agreed timescales. Review and act on investigation results and ensure that significant findings are communicated to patients and relevant clinicians.

Intercalated BSc or equivalent Higher degrees including MSc, PhD or MD (where the research thesis is not part of first medical degree)

Experience Essential

Evidence of experience in a range of acute and chronic cardiac cases Evidence of experience in a range of acute medical specialties, with experience of managing patients on unselected medical take during core training or equivalent Substantial NHS experience Significant GIM on-call experience

Desirable

Evidence of supporting the delivery of imaging (CMRI, TOE, stress echo, CTCA), Heart Failure, and general cardiology services according to skill set. Experience at CT/ST 1/2 level of managing patients with severe acute medical disease by the time of commencement of ST3 training.
