We are looking for an experienced and compassionate Health and Wellbeing Practitioner to provide clinical and operational leadership within the WE: Flourish team. This is an exciting opportunity for an experienced registered clinician who enjoys working across organisational boundaries, leading teams, improving services and supporting people with complex health and wellbeing needs.

Main duties of the job

As the Health and Wellbeing Practitioner, you will play a central role in ensuring WE: Flourish delivers safe, effective and person-centred support across Erewash. You will provide leadership and oversight of the service, supporting referral management, triage, quality assurance, multidisciplinary working, complex case support and service development.

Working closely with GP practices, Social Prescribers, Adult Social Care, VCSE organisations and wider health and care partners, you will help ensure people receive the right support at the right time whilst maintaining a strong focus on quality, safety and outcomes.

Job description Job responsibilities

Erewash Health Partnership is a single GP partnership providing core primary care services and support services across its constituent practices. EHP is also the prime provider for services delivered on behalf of Erewash Primary Care Network.

WE Flourish is a PCN-wide, non-emergency health and wellbeing service for adults aged 18 and over whose health and wellbeing are affected by appropriate social, emotional, practical, lifestyle or wider non-medical factors.

Job Summary

The WE: Flourish Health and Wellbeing Practitioner will act as Clinical and Service Manager, providing day-to-day clinical and operational leadership for the EHP/PCN-employed WE: Flourish team. The post holder will oversee the Single Point of Access (SPOA), support triage and tier allocation, assure Quality Conversations, coordinate complex case support and the weekly multidisciplinary meeting (MDM), and oversee outcome monitoring and discharge processes across the pathway.

The role will help ensure that WE: Flourish operates as a safe, coordinated and person-centred service for adults registered with an Erewash PCN practice. Clinical supervision and clinical decision-making are required; therefore, the post holder must hold an appropriate registered clinical qualification.

General Duties

At all times, the post holder will behave professionally, promote high-quality person-centred support, provide safe leadership and foster continual improvement across WE: Flourish, EHP and EPCN.

This job description is not intended to be exhaustive. The post holder will carry out other duties that may reasonably be required within the main purpose and level of the role, in response to changing service needs.

Key Job Responsibilities

Provide day-to-day clinical and operational leadership for the PCN-employed WE: Flourish team, in line with professional competence and registration requirements.

Oversee referral flow, tier allocation, workload and waiting-list management, ensuring referrals are reviewed through the SPOA and triaged within the service standard of three working days; provide allocation support where risk, complexity or uncertainty is identified.

Ensure people receive support according to need and intervention intensity: Tier 1 navigation and supported signposting; Tier 2 coaching, Social Prescribing or other supported intervention; and Tier 3 Quality Conversation, complex support, case holding and MDM discussion where required.

Provide line management, clinical supervision where appropriately qualified, professional support and day-to-day guidance to PCN-employed WE: Flourish roles.

Work closely with Erewash Voluntary Action (EVA) so that its commissioned Social Prescribing capacity is integrated into the WE: Flourish pathway, while recognising that EVA retains employment and line-management responsibility for Social Prescribing staff.

Support quality assurance of Quality Conversations, personalised support planning, case navigation and follow-up.

Support the weekly MDM, including preparation, case discussion, agreement of ownership and actions, documentation, patient communication and follow-up, while maintaining each practitioners and organisations professional responsibilities.

Maintain clear escalation routes for acute mental health crisis, safeguarding concerns, clinical concerns, requests for further senior clinical advice and operational concerns, in line with the agreed pathway.

Support performance monitoring, outcome reporting, patient feedback and evaluation.

Ensure consistent use of approved SystmOne referral, triage, Quality Conversation, MDM outcome, caseload and discharge documentation, with accurate coding and a clear audit trail.

Identify training needs and support staff development across personalised care, risk management, safeguarding, strengths-based conversations and health inequalities.

Support service development, pathway refinement and reporting into PCN governance arrangements.

Promote WE: Flourish across practices and partner organisations.

Monitor service risks and support mitigations, including demand, capacity, pathway delays and inconsistent patient experience.

Ensure the team works within agreed role boundaries and escalation routes, particularly in relation to clinical risk, mental health concerns and safeguarding.

Caseload, Pathway and Performance Oversight

Ensure case-holding arrangements follow the service specification: open cases reviewed at least every two weeks, normally held for no longer than three months, and transferred, discharged or escalated appropriately.

Oversee implementation of appointment confirmation, non-attendance, reassessment, transfer between tiers, discharge and re-referral processes, including risk review before discharge where safeguarding, vulnerability or significant concerns are known.

Support delivery across PCN/practice and agreed community locations by telephone or face to face within the service operating model.

Monitor referral volume, triage and Quality Conversation waiting times, tier distribution, caseload, case-holding duration, outcomes, re-referral, patient feedback, equity of access and demand against workforce capacity.

Communication and Relationships

Build effective working relationships across primary care, mental health, adult social care, public health, Social Prescribing, voluntary, community and social enterprise organisations and community partners.

Communicate complex or sensitive information clearly, compassionately and appropriately to people, carers, staff, clinicians, managers and partner organisations.

Promote person-centred, strengths-based and compassionate practice, focused on what matters to the person.

Recognise and respond to the effect of wider determinants of health, including housing, debt, isolation, bereavement, unemployment, caring responsibilities and low confidence.

Support equitable access by adapting communication and engagement approaches for people and communities who experience barriers to care.

Manage competing priorities, staff workflow and service risks, escalating concerns to the appropriate senior colleague or service.

Physical Skills

Standard keyboard and digital skills are required.

Use of EHP and PCN IT systems, including SystmOne.

Regular use of computer and telephone systems, alongside face-to-face work in practice and community settings.

Qualifications Essential

Appropriate registered clinical qualification, such as Registered Nurse or Occupational Therapist, where clinical supervision or clinical decision-making is required. And/or equivalent experience in health, care, mental health, personalised care or community-based support, with evidence of operating at senior practitioner level. Current professional registration and evidence of continuing professional development experience.
