Job summary
The Community GP is the senior clinical decision-maker within the TeamUp integrated neighbourhood team. This is a frontline clinical role. The post holder is expected to be visible, accessible and present across the service's clinical settings on a daily basis, providing the medical leadership and decision-making capacity that enables the wider team to function safely.
Main duties of the job
Attend the daily TeamUp situational awareness meeting: caseload review, new referrals, urgent work and capacity
Workload allocation: home visits, identification of patients requiring senior input, triage of acute referrals
Community visits, ward rounds, home visit triage and direct integrated neighbourhood team clinical support
Board round for patients under the service
Follow-up visits, clinical advice, medication review, governance tasks, supervision and training
Responsive availability for urgent clinical advice and team queries until end of operational day
About us
Erewash Health Partnership (EHP) was established on 1 April 2018 and is a single GP Partnership comprising of six constituent practices (see below). EHP employs approximately 250 people who work within the constituent practices and central team.
As well as providing high quality core primary care services, EHP is the prime provider for delivering services to Erewash Primary Care Network (EPCN), as well as operating its own services for EHP Partners.
Erewash PCN consists of 11 practices and covers a population of approximately 102 ,000 patients.
Job description Job responsibilities
Care Homes-
Holds clinical responsibility for patients on the TeamUp care home caseload
Attends the care home ward round and provides senior clinical input for patients discussed
Reviews and authorises investigations, prescriptions and medication changes for care home patients
Leads or contributes to Advance Care Planning and RESPECT form completion
Supports GSF identification and end-of-life planning
Works with the community pharmacist to support medication reviews and deprescribing
Provides oversight of monitoring for frail patients actively reducing unnecessary investigations where clinically appropriate
Leads or co-leads the fortnightly care home integrated neighbourhood team meeting as the GP representative
Housebound Patient Care
Provides senior medical assessment for housebound patients with acute or complex needs
Contributes to development of personalised care and support plans
Supports structured annual reviews for housebound patients on the TeamUp caseload
Acute Home Visiting Service Triage and Support
Triages referrals from GP practices and community teams to the Acute Home Visiting Service
Assesses urgency and clinical suitability for community management
Allocates visits to appropriate clinicians where a presentation is unsuitable for AHVS
Provides real-time telephone or remote clinical advice to ACPs and paramedic practitioners during acute visits
Attends acute home visits directly where clinical complexity requires GP-level assessment
Person Specification
Experience Essential
Understanding of adult safeguarding and Deprivation of liberty procedures. Experience of senior clinical triage for the to determine the urgency and type of response needed, according to clinical need. Experience of using SystmOne. Experience of medical education/clinical supervision
Desirable
Knowledge and experience of carrying out Comprehensive Geriatric Assessment. Experience of working in a multi-disciplinary team.
Qualifications Essential
GMC Registration Full registration with valid licence to practise must be maintained NHS Performers List Must be maintained CCT in General Practice Required Significant previous experience in elderly care, frailty, community services or relevant acute care speciality
Desirable
Recognised qualification in medical education/clinical supervision Evidence of leadership development
