About this role

Job summary

As an Advanced Nurse Practitioner, you will work as an autonomous practitioner, assessing, diagnosing, treating and managing patients presenting with a wide range of acute, chronic, undifferentiated and undiagnosed conditions and, where appropriate, discharging patients from your care.

You will see patients across all age groups, including young children, through face-to-face appointments, telephone consultations and other appropriate methods of clinical assessment.

You will practise within your professional competence and scope of practice, in accordance with NMC standards and the practice's clinical governance arrangements, using advanced clinical skills to provide safe, effective and evidence-based care.

Main duties of the job

Undertake comprehensive clinical assessments and consultations.Assess, diagnose, and manage patients presenting with a broad range of acute and chronic conditions.Manage patients with undifferentiated and undiagnosed presentations.Prescribe safely and appropriately within your scope of practice.Refer patients to secondary care and other appropriate services where required.Provide appropriate treatment, follow-up, safety-netting and clinical advice.Recognise and manage patients whose condition requires urgent escalation or onward referral.Work autonomously while recognising when advice or support from colleagues is required.Maintain accurate and contemporaneous clinical records.Contribute to clinical governance, audit, service development and continuous improvement within the practice.Support effective multidisciplinary working and communication.Promote health, wellbeing and self-management where appropriate.Work collaboratively with colleagues to ensure patients receive safe, coordinated and compassionate care.

Job description Job responsibilities

The successful post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.

The following are the core responsibilities of the ANP in delivering health services.

a.Develop, implement and embed health promotion and wellbeing programmes

b.Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

c.Implement and evaluate individual specialised treatment plans for chronic disease patients

d.Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

e.Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

f.Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children

g.Request and collect pathological specimens. Upon receipt, to process and interpret pathology and other test results as required

h.Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

i.Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectively

j.Maintain chronic disease registers and implement and embed an effective call/recall system

k.Develop, implement and embed well woman and well man clinics

l.Chaperone patients where necessary

m.Prioritise health issues and intervene appropriately

n.Support the team in dealing with clinical emergencies

o.Recognise, assess and refer patients presenting with mental health needs

p.Support in the provision of vaccination programmes for both adults and children

q.Be an extended and supplementary prescriber, adhering to extant guidance

r.Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

s.Contribute to practice targets (QOF, etc.), complying with local and regional guidance

t.Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)

u.Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)

v.Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

w.Deliver opportunistic health promotion where appropriate

There may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels.

Wider Responsibilities:

In addition to the primary responsibilities, the ANP has the following wider responsibilities:

a.Provide holistic support in the clinical governance spectrum assuming a senior role in its compliance

b.Support the IPC lead for the practice, ensuring compliance

c.Monitor and ensure the safe storage, rotation and disposal of medications

d.Undertake overall management of the nursing team, providing guidance when necessary, acting as a mentor to students and newly qualified staff

e.Participate in local initiatives to enhance service delivery and patient care

f.Support and participate in shared learning within the organisation

g.Develop an area of specialist interest, taking the lead within the organisation

h.Continually review clinical practices, responding to national policies and initiatives where appropriate

i.Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required

j.Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation

Qualifications Essential

Registered Nurse with current registration with the Nursing and Midwifery Council (NMC). Qualified and experienced Advanced Nurse Practitioner, educated to Masters level in Advanced Practice or able to demonstrate recognised Masters-level equivalence in advanced practice, consistent with the NHS England Multi-professional Framework for Advanced Practice. Independent/Supplementary Nurse Prescriber (V300 or equivalent), with prescribing qualification recorded with the NMC. Evidence of advanced-level autonomous clinical practice, including comprehensive history taking, clinical examination, assessment, diagnosis and management of patients with undifferentiated and differentiated presentations. Demonstrable competence in the assessment and management of minor illness and acute presentations. Ability to formulate differential diagnoses, request and interpret appropriate investigations, initiate treatment and appropriately refer/escalate patients. Evidence of working within an appropriate scope of practice and recognising professional and clinical limitations. Evidence of continuing professional development and compliance with NMC revalidation requirements. Excellent communication, clinical reasoning, decision-making and risk-management skills. Ability to work autonomously while contributing effectively to a multidisciplinary primary care team.

- Previous experience working as an ANP/Advanced Practitioner in general practice or primary care. - Experience managing same-day/urgent primary care presentations. - Experience of telephone and/or digital clinical triage. - Experience in management of long-term conditions. - Advanced clinical assessment/examination qualification. - Teaching, mentorship or clinical supervision qualification/experience. - Basic life-support training. - Experience using EMIS Web or another primary-care clinical system. - Understanding of primary care governance, QOF, medicines optimisation, safeguarding and referral pathways. - Knowledge of the structure of NHS primary care, including general practice, PCNs, ICBs and community services.

Clinical Knowledge and Skills Essential

Advanced clinical assessment, history-taking, examination and clinical reasoning/differential diagnosis skills. Ability to autonomously assess and manage patients presenting with undifferentiated and acute/minor illness. Independent prescribing skills, with sound knowledge of pharmacology, medicines optimisation and safe prescribing. Ability to request and interpret appropriate investigations, formulate management plans, refer appropriately and provide effective safety-netting. Ability to recognise red flags and the deteriorating patient, including appropriate escalation to GP, secondary care or emergency services. Broad knowledge of evidence-based practice, NICE guidance, clinical governance and adult and child safeguarding. Ability to work autonomously within own scope of practice, maintain accurate clinical records and use primary-care clinical systems effectively.

Previous experience of managing same-day/urgent presentations in general practice or primary care. Experience using EMIS Web or another primary-care clinical system.
