About this role
Job summary In this role, you'll be trusted to assess patients and deliver safe, personalised nursing care in their homes. You'll work within the District Nursing Service, helping people maintain choice, independence and control over their health. Working both independently and alongside a wider care team, you'll respond to changing needs and support patients, families and carers. You'll receive professional support while developing your clinical, leadership and mentoring skills.
Main duties of the job What you'll do:
Complete holistic assessments and develop, deliver and review personalised care plans Provide safe clinical care, including treatments appropriate to your skills and professional practice Recognise changes in patients' needs and share relevant information with colleagues and partner services Maintain accurate, confidential records and report clinical incidents appropriately Support the learning and development of junior colleagues and students What we're looking for: Registered Nurse status and relevant professional registration Healthcare experience, including supporting patients, families and carers Knowledge of patient assessment, evidence-based practice and safe care Clear communication, teamwork, organisation and basic digital skills Flexibility to meet service needs and travel between patient visits At CHCP, we welcome different backgrounds, experiences and ideas, and treat everyone with fairness and respect.
